肝切除围手术期营养治疗难点与实施要点

摘要:

肝切除围手术期营养治疗的方法、途径、制剂均有其特殊性。其实施要点主要有以下几个方面:(1)术前营养评估和治疗。对存在营养风险或营养不良的病人应从术前开始营养治疗。(2)术后早期肠内营养。坚持“先肠内、后肠外、肠内不足肠外补”原则。(3)适量补充白蛋白,维持血浆胶体渗透压。(4)围手术期液体管理。根据液体出入量和体重的变化,适当使用利尿剂,调控围手术期的液体平衡。

Abstract:

Abstract    The methods, approaches and preparations of perioperative nutrition therapy for hepatectomy have their particularity. The main points of its implementation are as follows: (1) Preoperative nutrition assessment and treatment. Patients with nutritional risk or malnutrition should begin the preoperative nutritional therapy. (2) Early postoperative enteral nutrition. Adhere to the "first bowel, after the intestine, intestinal insufficiency of the external supplement" principle. (3) The appropriate albumin supplementation is helpful to maintain plasma colloid osmotic pressure. (4) Perioperative fluid management. According to the fluid intake and weight changes, the appropriate use of diuretics is needed to regulate perioperative fluid balance.
