机器人胃癌根治术难点与技术要领

摘要：

近年来，应用机器人手术系统行胃癌根治术在国内逐渐得到开展，虽然机器人手术系统具有三维立体视野、机械臂操作灵活等优势，但与传统腹腔镜胃癌手术不同，应用机器人手术系统行胃癌根治术有其特有的难点和技术操作要领，如trocar布局，脾门区、幽门上区和胰腺上缘区域的淋巴结清扫以及消化道重建等。建议采用机器人胃癌手术“W”形五孔法的trocar布局，合理使用3号臂协助暴露以及机械和手工缝合相结合的吻合方法等应对策略。

Abstract    In recent years，more and more robotic radical gastrectomy for gastric cancer has been carried out  in China. Although the robotic surgery system with three-dimensional vision，the manipulator is flexible and other advantages, the robotic gastrectomy has its own difficulties and technical essentials，which is different from the traditional laparoscopic surgery for gastric cancer，for example, the site of trocas，dissection of lymph node, and reconstruction of digestive tract. It is suggested that resolving methods should include the "W" type troca site, using No.3 arm to expose and anastomosis with machine and hand sewing methods.
