Abstract:

Value and evaluation of associating liver partition with portal vein ligation for staged hepatectomy（ALPPS）in the treatment of colorectal liver metastases        ZHENG Shu-guo.Insitute of Hepatibiliary Surgery, Southwest Hospital, the Third Military Medical University ,Chongqing 400038,China
Abstract    As a new surgical procedure, associating liver partition with portal vein ligation for staged hepatectomy （ALPPS） has provided opportunities of curative resection for many of the unresectable multiple or huge liver cancer patients with insufficient remaining liver volume. It has been showing a magical effect and broad application prospects, which caused widespread concern and controversy in the international hepatobiliary surgical field. Although the indications for ALPPS continue to expand, colorectal liver metastases is still the best indication for the procedure. By selecting the indications strictly for surgery and improving technology continuously, especially by the application of minimally invasive technology, such as laparoscopic surgery, robotic surgery and radiofrequency ablation, perioperative morbidity and mortality of ALPPS have been reduced significantly. Existing studies have shown that the using of ALPPS in the treatment of colorectal liver metastases is safe and feasible with good long-term results of oncology, but the outcomes still needs a large case series, prospective randomized controlled study for further confirmation. Currently, a consensus on the indications and contraindications for ALPPS and a standardized operative protocol are needed.
