腹腔镜手术与开腹手术治疗胃肠间质瘤对比Meta分析

摘要:

目的    从循证医学角度评价腹腔镜胃肠间质瘤切除术(LAP)的安全性和有效性。方法    收集2004年1月至2014 年 3月发表的有关LAP 和开腹胃肠间质瘤切除术(OPEN)对比的英文文献,按纳入和排除标准筛选文献、提取资料并进行质量评价后,利用Review Manager 5.2软件进行Meta 分析。结果    共纳入12篇文献(719例病人),其中LAP组345例,OPEN组374例。荟萃分析结果显示,与OPEN相比,LAP术后排气时间、进流质时间和住院时间更短(均P<0.05),术后并发症发生率及复发率更低(均P<0.05),而手术时间、术中出血量差异均无统计学意义(均P>0.05)。结论    行LAP治疗胃肠间质瘤安全可行,且具有良好的近期疗效。

Abstract:

Abstract    Objective    To evaluate the safety and efficacy of laparoscopic-assisted resection(LAP) for gastrointestinal stromal tumor(GISIs)by systematic review and meta-analysis. Methods    The English literature comparing LAP and open resection (OPEN) for GISIs publicly published from January 2004 to March 2014 were searched from several international online databases. After screening for inclusion,data extraction,and quality assessment,Meta-analysis was conducted by the Review Manager 5.2 software. Results    There are Twelve studies meeting the inclusion criteria for meta—analysis.The total sample size of these studies was 719 cases,of whom,345 cases underwent LAP and 374 cases were subjected to OPEN.Compared to OPEN,LAP not only experienced shorter time of postoperative flatus(P=0.04),liquid(P<0.05) and hospital stay(P<0.05),but also experienced less postoperative complications(P<0.05) and recurrence rate(P<0.05). The number of operative time and intraoperative blood loss were similar between the two groups(P>0.05).Conclusion    LAP is a safe and feasible procedure and has a good short-term effect.
