高海拔地区失血性休克早期限制性液体复苏临床研究

摘要:

目的 探讨高海拔地区失血性休克早期限制性液体复苏的临床意义。方法 2001年12月至2003年12月根据不同的复苏方式将41例失血性休克病人分为对照组和实验组,检测血红蛋白(HB)、血小板计数(PLT)、红细胞压积(HCT)、血清乳酸水平和失血量变化情况,比较限制性液体复苏与常规液体复苏对高海拔地区失血性休克病人的疗效。结果 不同处置后12h两组 HCT变化差异有显著性意义(P<005);12h及24h两组PLT变化差异有显著性意义(P<005);24h两组血清乳酸变化差异有显著性意义(P<005)。结论 高海拔地区失血性休克病人早期液体复苏时,限制液体复苏相对于快速大量液体复苏不至于扰乱机体的代偿机制和内环境,且对改善细胞氧代谢更有利。

Abstract:

Clinical Research of Early Restriction Resuscitation in Treatment of Hemorrhagic Shock at High Altitude. Ma Wenbiao,Gongbao Caidan,Chen Guangan,et al.The Urgent Care Centre of Qinghai Province People s Hospital,Qinghai 81007,China Abstract Objective To investigate the clinical meaning of early restriction resuscitation in treatment of hemorrhagic shock at high altitude. Methods According to different resuscitation modes,there are comparison group(rapidly large fluid resuscitation) and experiment group (fluid restriction resuscitation).By examining HB,PLT,HCT,serum lactate and quantity of the lose blood at different time,the researchers compare the therapeutic effect of the hemorrhagic shock patients at high altitude with fluid restriction resuscitation and fluid normal resuscitation. Results ①At the 12th hour the comparison of the two groups’ HCT is significantly different (P<005).Between the 12th hour and 24th hour the comparison of the groups’ PLT is significantly different (P<005).It shows that the fluid restriction resuscitation group can decrease hemorrhage through to prevent blood from attenuating and to steady blood clotting function.②At 24th hour the researchers compare serum lactate of two groups and the difference is significant (P<005).It shows that the fluid restriction resuscitation group can effectively improve the organizing oxygen infusion and alleviate the acid-intoxication during the shock period. Conclusion At high altitude the hemorrhagic shock patients with fluid restriction resuscitation can obviously improve cell oxymetabolism and steady internal environment.
