胃肠胰神经内分泌肿瘤病理诊断共识与争议

摘要:

第4版《世界卫生组织消化系统肿瘤分类》发布了2010年版胃肠胰神经内分泌肿瘤(gastroenteropancreatic neuroendocrine neoplasm,GEP-NENs)新分类。根据新分类,2011年《中国胃肠胰神经内分泌肿瘤病理学诊断共识》(下文简称《中国共识》)发表,成为首个统一中国GEP-NENs病理诊断名称、分类和分级,规范GEP-NENs病理诊断的指导性意见。随后几年有关GEP-NENs的临床研究获得了一些重要数据和发现,对GEP-NENs的分类、分级系统提出了新见解。根据近年来在GEP-NENs方面新的临床研究结果和实际工作中遇到的具体问题,2013年发表的对《中国共识》的更新,进一步强调了一些在GEP-NENs常规病理诊断时的要点,补充了不同分化程度时典型的GEP-NENs形态特点描述,介绍了增殖活性指数计数方法。针对尚存争议的分化中等的G3肿瘤,2013年更新的《中国共识》也提出了自己的观点和建议,有待于临床和病理的进一步研究证实。

Abstract    Revised classification, nomenclature and grading system of gastroenteropancreatic neuroendocrine neoplas (GEP-NENs) was proposed by the fourth edition World Health Organization Classification of Tumours of the Digestive System in 2010. Chinese Consensus of Pathological Diagnosis of Gastroenteropancreatic Neuroendocrine Neoplasm (hereinafter referred to as Chinese Consensus ) was issued in 2011. It apparently contributes to unify the methods of classification,put an end of the confusion in nomenclature, standardize the grading system and predict the outcomes more accurately. Recently, new advances in GEP-NENs study put forward a new view in the classification and grading system that leading to several controversies. Consequently, an update of Chinese Consensus was published in 2013. It not only emphasized the key points of pathological diagnosis of GEP-NENs, expanded the content of morphological description and the methods of proliferation index evaluation, but also made its own suggestions aimed at the pending problem. However, the update of system needs further studies.
