摘要: 复发疝的修补材料选择直接影响手术效果与预后。复发性腹股沟疝应避免原手术路径,优先选用聚丙烯补片,生物补片仅限潜在感染时慎用;复发性切口疝需强化力学重建,推荐中/重量型合成补片,生物补片因高复发率及并发症风险不推荐使用;复发性造口旁疝建议采用Sugarbaker术式结合大网孔合成补片(如聚丙烯),生物补片疗效欠佳;复发性食管裂孔疝需补片加强,缺损较大时优选合成补片。需结合疝类型、缺损特点及病人状况个体化选择补片材料,且当前补片选择相关临床证据不足、样本量有限。未来应聚焦智能补片的研发,通过仿生设计、动态力学响应及实时监测技术,推动疝修补向精准化、功能化发展。

Abstract: The choice of prosthetic materials for recurrent hernia repair directly impacts surgical outcomes and prognosis. For recurrent inguinal hernias, the original surgical approach should be avoided, with polypropylene mesh being the preferred choice, while biological mesh should only be considered cautiously in cases of potential infection; for recurrent incisional hernias, mechanical reinforcement is essential, and medium/heavyweight synthetic meshes are recommended, whereas biological meshes are not advised due to their high recurrence rate and complication risks; for recurrent parastomal hernias, the Sugarbaker technique combined with large-pore synthetic mesh (e.g., polypropylene) is suggested, as biological meshes demonstrate suboptimal efficacy; for recurrent hiatal hernias, mesh reinforcement is required, with synthetic meshes preferred for larger defects. The selection of patch materials should be individualized in combination with the type of hernia, the characteristics of the defect and the patient's condition. Moreover, the current clinical evidence related to patch selection is insufficient and the sample size is limited.Future directions focus on the development of smart meshes, leveraging bionic design, dynamic mechanical responsiveness, and real-time monitoring technologies to advance hernia repair toward precision and functional optimization.
