一种切口疝缝合的创新技术——“立体”缝合

摘要: 普通的缝合可视作以进针点和出针点作为着力点,使伤口对合的一种“平面”缝合。这种缝合方法一直应用在切口疝的缝合中,但存在诸多问题。缝合材料的形态学进展促进了缝合技术和缝合方法的创新。通过鱼骨线上的突起,可改变缝合的受力点,如“齿轮”的齿一样,通过连续缝合,可形成“齿轮”的组合,如“变速箱”样的结构,收紧缝线即可收缩空间,称为“立体”缝合。将“立体”缝合应用至切口疝的缝合中,可有效消除疝囊空间,确切关闭筋膜缺损,同时恢复腹壁厚度,在临床实践中取得满意效果。“立体”缝合技术将有可能为切口疝的外科治疗提供一种更安全、更有效的方法。

Abstract    The conventional suture can be regarded as a kind of "plane" suture in which the point of entry and exit of needle bear the force to make the wound close. The method has been used in suture of the incisional hernia, but there are many problems. The morphological progress of suture materials promotes the innovation of suture technology and suture method. Just like the fishbone protuberance on the Stratafix Symmetric, it can change the stress point of suture, such as the wheels of "gear". Through continuous stitching, the combination of "gears" can be formed, such as the structure of "transmission", and the space can be reduced by tightening the suture. The technique was called "multidimensional" suture. The application of such technique in suture of incisional hernia can effectively eliminate the space of hernia sac, accurately close the fascia defect and restore the thickness of abdominal wall. In the clinical practice, satisfactory results have been achieved. The "multidimensional" suture technique will potentially provide a safer and more effective method for surgical treatment of incisional hernia.
