改良Appleby手术治疗局部进展期胰腺癌有效性探讨

摘要：

既往改良Appleby手术对进展期胰腺癌疗效不佳。随着新辅助放化疗的应用和手术技术的进步，结合切除范围的适当扩大，局部进展期胰体尾癌病人实施改良Appleby手术的R0切除率明显提高，术后病人远期生存时间可得到大幅延长。但目前仍仅应在大型胰腺外科中心开展此项手术，且术前须选择合适的病人并经过多学科充分讨论。

Abstract:

Abstract    Modified Appleby procedure used to show unsatisfying therapeutic effect in treatment for locally advanced pancreatic cancer. Along with applications of neoadjuvant therapy，advances of surgical techniques，and proper determination of excision extension，modified Appleby procedure improves R0 rate in locally advanced cancer in the body and tail of the pancreas，with a better long-term survival.Currently, the Appleby procedure should be performed in large pancreatic centers for carefully selected patients with preoperative multidisciplinary discussion.
