自体牙移植术及围手术期处理

  • 作者简介:周宏志，医学博士、副教授、副主任医师。2001年于第四军医大学获博士学位。2002—2004 年于中国人民解放军总医院完成博士后工作。2010 年赴美国马里兰大学牙科学院做访问学者。现工作于第四军医大学口腔医学院口腔外科。兼任中华口腔医学会口腔颌面外科专业委员会牙槽外科学组委员。从事口腔颔面外科临床和研究工作16年，在牙槽外科、组织工程等方面有深入研究。曾主持中国博士后科学基金、北京自然科学基金、军队科学研究基金等项目。近年来，在SCI收录国际期刊发表论文10篇，国内核心期刊发表论文30余篇，参编专著4部。获解放军总医院科技进步奖1项、国家发明专利l项。

摘要：

在满足适应证条件下，自体牙移植是很多病例值得尝试的选择，良好的牙移植术及围手术期处理会取得超过90%以上的成功预期，自体牙移植术的操作过程应该注意采用微创方法拔除患牙、预备受区和植入供牙，并且术前检查和准备应做到有备无患，术后应采用合理方式固定以及根据牙根发育情况进行根管治疗等，以保证移植牙实现牙周韧带愈合和长期保有良好功能。

Abstract:

Autotransplantation of teeth could be a preferable treatment for many patients with appropriate indications. Good tooth-transplant surgery and perioperative management would achieve a success rate of more than 90%. The key points of the teeth autotransplantation include atraumatic extraction of the teeth, minimally invasive preparation of alveolar socket, and proper implantation of the donor tooth. Furthermore，the operator should also make a cautious preoperative examination and preparation. And postoperative treatments, including a suitable fixation of tooth crown and a reasonable handling of root canal, would be important as well to ensure the periodontal ligament healing and long-term function maintaining of the transplanted teeth.
