山东地区不同错牙合患者Bolton指数测量分析

摘要: 目的    分析山东地区各类错牙合患者Bolton指数及Bolton指数不调的发生率。方法  选取2005—2009 年山东省口腔医院及济南市口腔医院正畸科收治的180例错牙合畸形患者的石膏模型,按Angle错牙合分类法分为AngleⅠ、Ⅱ、Ⅲ 3组,每组60 例。测量并分析Bolton指数。结果    山东地区不同错牙合患者的Bolton指数前牙比、全牙比在性别及错牙合类型方面的差异无统计学意义(P>0.05),与中国人正常牙合相比差异亦无统计学意义(P>0.05)。以中国人正常牙合Bolton指数±2个标准差为标准,山东地区错牙合患者的前牙比、全牙比不调的发生率分别为14.44%、6.66%,且不同类型错牙合间差异无统计学意义(P>0.05)。以︳Bolton不调量(TSD)︳≥1.5 mm为标准时,Bolton指数前牙比、全牙比不调的发生率分别为17.23%、40.00%,Angle Ⅰ类错牙合全牙比不调的发生率与Angle Ⅲ类错牙合相比差异有统计学意义(P<0.05)。结论    中国人正常牙合的Bolton指数比值同样适用于山东地区各类错牙合患者;与Bolton指数的百分比值相比,TSD的实际值可以为临床正畸医生提供更多有关牙量不调的信息。

Abstract: Objective    To analyze the Bolton index ratios and Bolton’s tooth size discrepancy frequency in different malocclusions in Shandong province. Methods    Bolton indexes were measured and analyzed in randomly chosen 180 dental casts of three Angle’s malocclusions by the method described by Bolton. Results    The Bolton anterior and overall ratios for different malocclusions of Shandong province showed no signi?cant differences in anterior or overall ratios among different occlusal categories or between the different sexes,and no statistically signi?cant differences for every malocclusion when compared to the Chinese normal occlusion’s Bolton index ratios. The anterior and overall tooth size discrepancies frequency outside two standard deviations from the Chinese normal occlusion’s Bolton index ratios were 14.44% and 6.66% for all the malocclusions,respectively,and there were no signi?cant differences in anterior or overall tooth size discrepancies frequency among the three Angle’s malocclusions. When more than 1.5 mm Bolton tooth size discrepancy(TSD)expressed in millimeters was used,the anterior and overall tooth size discrepancies frequency for all the malocclusions were 17.23% and 40.00%,respectively,and there were no signi?cant differences in anterior or overall tooth size discrepancies frequency among the three Angle’s malocclusions,except the overall tooth size discrepancy between Class Ⅰ and Class Ⅲ malocclusions. Conclusion    The Chinese normal occlusion’s Bolton index ratios can be also used to malocclusions in Shandong province. The actual amount of discrepancy expressed in millimeters provides orthodontists with more useful information for clinically signi?cant tooth size discrepancy than the ratio in percentage terms.
