牙科患者局部麻醉前评估

  • 作者简介:周宏志，医学博士、副教授、副主任医师。1995年毕业于第四军医大学获学士学位，2001年获博士学位。2002—2004年于中国人民解放军总医院完成博士后工作。2010年赴美国马里兰大学牙科学院做访问学者。现工作于第四军医大学口腔医学院口腔外科。兼任中华口腔医学会口腔颌面外科专业委员会牙槽外科学组委员。从事口腔颌面外科临床和研究工作16年，在牙槽外科、组织工程等方面有深入研究。曾获中国博士后科学基金、北京自然科学基金、军队科学研究基金等资助。发表论文40余篇，其中SCI收录论文10篇。参编专著4部。获解放军总医院科技进步奖1项，国家实用新型专利l项。

摘要： 局部麻醉（简称“局麻”）是无痛治疗的基本手段，有效的局麻是许多牙科门诊工作顺利、安全进行的重要保证，但局麻针刺操作和注射药物可能对患者精神和肌体产生不良作用。局麻前进行个体化评估是保证麻醉有效性和避免局麻意外的必要措施，局麻前评估内容一般包括患者年龄、体重、健康状况、精神心理状况、麻醉史、麻醉方法和药物选择、局麻意外应急预备等。全面准确的评估及相应准备可确保局麻效果、避免或减少麻醉风险，是每个牙科医生都应具备的理念和能力。

Abstract: Local anesthesia is the basic method of pain-control for many procedures in dental treatment. But there are a number of complications that can occur following the use of local anesthetics in the orofacial region. Local or systemic complications can arise as the results of drug allergy，drug toxicity，drug interaction and patients' anxiety. The goal of evaluation before local anesthesia is to reduce the patient risk and improve the anesthesia outcome. The individual evaluation includes：age，weight，health，anxiety，history of anesthesia，instruments and drugs for local anesthesia，possible emergency accidents and their managements. A comprehensive consideration of these conditions could help dentists to avoid failure of local anesthesia and risks of local and systemic complications.
