上颌活动矫治器联合前方牵引治疗替牙期反牙合26例疗效评价

摘要：

目的    评价上颌活动矫治器联合前方牵引治疗替牙期反牙合的疗效。方法    选择2006年3月至2010年10月在溧阳市中医院口腔正畸门诊就诊的替牙期反牙合患儿26例，采用上颌活动矫治器联合前方牵引进行矫治，矫治前、后均拍摄头颅侧位片并对X线头影测量结果进行分析。结果    26例患儿经4 ~ 8个月矫治后，前、后牙反牙合均全部解除，第一恒磨牙为中性或偏近中关系，前、后牙覆牙合及覆盖正常，侧貌明显改善。除MP-FH角（由下颌平面与眼耳平面的交角）外，其他X线头影测量指标的矫治前后差异均有统计学意义（P < 0.05）。结论    上颌活动矫治器联合前方牵引能够促进上颌骨生长，抑制下颌骨向前生长，显著改善患儿面型，是替牙期反牙合早期矫治的有效手段。

Abstract:

Objective    To evaluate the correction effect of using maxillary protraction to cure crossbites of dental transitional period with the help of maxillary removable applicance. Methods    Totally 26 children who visited the Orthodontic Clinic of Traditional Chinese Medicine of Liyang city from Mar. 2006 to Oct. 2010 for crossbites of dental transitional period were chosen as subjects. The method of employing maxillary protraction with the help of maxillary removable applicance was used to cure them. Lateral cephalograms were taken before and after the treatment. The results of the cephalometric X-ray were analyzed. Results    The anterior crossbites and posterior crossbites of the 26 cases were all corrected after four to eight months’ treatment. The first permanent molar was neutral or mesial. The coverage of the anterior overbites and the posterior overbites was normal，and the profile was greatly improved. In addition to the MP-FH angle，the differences of the cephalometric X-ray index before and after the treatment were statistically significant（P < 0.05）. Conclusion    The method of employing maxillary protraction with the help of maxillary removable applicance can promote the growth of maxillary bone， inhibit the forward growth of mandible，and consequently，can improve the face type of the patients. It is an effective way to cure the early crossbites of dental transitional period.
