插销式桩冠应用于咬合过紧患者磨牙残根残冠修复临床研究

摘要:

目的    评价插销式桩冠修复咬合过紧患者磨牙残根残冠的临床疗效。方法    随机选择2006—2007年来佛山市禅城区口腔医院修复科就诊的22例咬合过紧患者的29颗磨牙残根、残冠,采用插销式桩冠修复后随访3年。随访内容包括桩冠是否松动脱落、是否折断、是否根折、牙龈状况、桩冠边缘密合度、食物嵌塞和继发根尖周炎等。结果    观察期间失访1例患者,其余21例患者的28颗患牙中,有2颗患牙在修复后1年复诊时出现轻微牙龈炎症,余牙未见异常。在修复后2年复诊时所有患牙未见异常。在修复后3年复诊时有1颗患牙牙根折断,出现牙周脓肿,需要拔除;另有2颗患牙稍有食物嵌塞,出现轻微牙龈炎症,余牙未见异常,咀嚼功能良好。结论        插销式桩冠在咬合过紧患者磨牙残根、残冠的修复方面具有独特优势,可在临床上推广应用。

Abstract:

Objective    To evaluate the clinical effect of bolt post and crown in restoration of residual root and crown in patients with tight occlusion. Methods    Twenty-nine destruction teeth of twenty-two patients with tight occlusion were collected randomly and restored with bolt post and crowns in the Stomatological Hospital of Guangdong Foshan Chancheng from 2006 to 2007. The effectiveness of these prostheses was followed up for 3 years. The contents of the follow-up included the loss of the bolt post and crown,fracture of the bolt post and crown,root fracture,gingival health,marginal fitness of the bolt post and crown,food impaction and secondary periapical periodontitis. Results    Twenty-one patients with 28 restored teeth including 20 second mandibular molar and 8 first mandibular molar completed the recheck treatment and one patient missed in the follow-up period. Slight gingival inflammation appeared only in 2 teeth at the 1-year follow up. All teeth were normal at the 2-year follow up. One tooth was extracted because of root fracture and periodontal abscess. Slight food impaction and gingival inflammation appeared in 2 teeth while the rest were normal with good mastication. Conclusion    The bolt post and crown has distinctive advantage on restoration of residual root and crown in patients with tight occlusion,and is a reliable treatment option.
