模型处理方法对烤瓷冠桥边缘密合度影响的实验研究

摘要: 摘要:目的 探讨2种不同的模型处理方法对烤瓷冠桥边缘密合度的影响。方法 采用156颗天然离体牙作为基牙,按修复设计原则制成各类冠、桥修复的牙列模型共55件(单冠5件、双端固定桥25件、复合固定桥25件)。把制作好的牙列模型固定在口腔临床模拟系统上,分别请10位不同年资的医生按要求进行牙体预备,基牙肩台的制作形式有90°、90°+斜面、135°、凹形等常用形式,然后用硅橡胶制取印模,灌注硬石膏模型。把模型随机分为2组:采用边缘精密修整法的实验组29件,基牙共82颗;采用现行常规方法的对照组26件,基牙共74颗。对两组修复体分别采用肉眼观察法、探针法、10倍牙科实体显微镜观察法进行边缘密合度检查。结果 实验组和对照组所有修复体均能顺利就位,采用肉眼观察法,边缘密合度为100%,修复效果一致;探针法检查实验组密合度为93.9%,对照密合度为85.1%,修复效果差异有统计学意义(P < 0.05);10倍牙科实体显微镜观察法实验组密合度为90.2%,对照组密合度为66.2%,修复效果差异有统计学意义(P < 0.01)。结论 采用边缘精密修整法制作的修复体边缘在微观检测下仍保持较高的密合度,不同的模型处理方法对烤瓷冠桥边缘密合度具有较大的影响,在口腔技工操作中应引起重视。

Abstract: Abstract:Objective To study how the two different methods affect marginal adaptation efficiency of porcelain-fused-to-metal crown and fixed bridge. Methods A total of 156 in vitro teeth were used as abutment tooth and then, according to repair design principles, make 55 dentition models of different repaired crowns and bridges,including 5 single crowns, 25 rigid fixed bridges,and 25 compound fixed bridges. Fix the ready-made dentition models onto the oral clinical simulation system, and then, invite 10 dentists with different years of service to make tooth preparation accordingly. The ways of making abutment tooth shoulders involved those commonly-used forms such as 90°, 90° plus bevel, 135°, chamfer, etc. Finally,make impressions with silicone rubber and fill models with superhard stone. Randomly, categorize all models into two groups: 29 pieces for experiment in Group A with 82 abutment teeth,based on the methods mentioned here; 26 pieces for contrast in Group B with 74 abutment teeth, based on the conventional methods. Results By examining the marginal adaptation efficiency of the two groups of prostheses by means of naked eye observation, probe and dental microscope observation,we noticed that the two groups of prostheses were able to smoothly adapt to their places,with the results of marginal adaptation efficiency of 100% in naked eye observation for the two groups, 93.9% in Group A and 85.1% in Group B in probe method(P<0.05),and 90.2% in Group A and 66.2% in Group B in dental microscope observation(P<0.01). Conclusion The repair marginal adaptation efficiency in the method mentioned here is superior to those in the conventional methods. Thus, different model preparations have a striking effect on the marginal adaptation efficiency of porcelain-fused-to-metal crown and fixed bridge, which should be paid much attention to in the application of oral technology for the purpose of reducing the risk of complications after the repair and prolonging the use of prostheses.
