牙周病伴牙列缺损套筒冠修复疗效分析

摘要:

目的 探讨以圆锥形套筒冠为固位体的可摘局部义齿用于牙周病伴牙列缺损修复的治疗效果。 方法 从山西省晋城煤业集团总医院口腔科艾坚口腔管理系统(V3、V4)中选择2003年1月至2008年12月的牙周病伴牙列缺损并应用套筒冠可摘义齿进行修复患者38例(男24例,女14例;病历资料完整,随访3年以上),共204个套筒冠单位(颗基牙)。对患者治疗后的主观感觉、功能改善、牙周情况等进行分析。结果 患者经修复治疗1个月后,均表示对义齿的外观和舒适度满意,咀嚼功能改善明显,对说话无明显影响;半年后修复体使用正常,功能及舒适度满意。患者修复治疗后,牙周袋逐渐消失或变浅[(4.0±0.5)mm对(1.1±0.5)mm],牙齿松动度明显改善,治疗前后比较,差异均具有统计学意义(P < 0.05)。有根分叉病变的牙齿有新的附着形成,X线片显示基牙的牙周膜和骨硬板清晰完整,牙槽骨边缘高度未见再降低,骨密度明显增加,除个别牙齿有牙龈缘的轻微炎症和附着丧失、萎缩外,所有修复体均应用良好。在观察期内修复体成功率达100%,基牙的保存成功率达100%。结论 套筒冠义齿修复是一种预期良好、可靠、容易制作且舒适的方法,对牙周病伴牙列缺损的修复治疗疗效肯定。

Abstract:

Objective To retrospectively study the outcome of treatment with telescopic crown-retained removable denture for dentition defect with periodontitis. Methods This study was performed between January 2003 and December 2008. All patients were in the Stomatology Department of Jincheng Anthracite Mining Group General Hospital:38 patients(24 men and 14 women) with periodontal diseases were treated with 44 telescopic dentures(30 in the mandible and 14 in the maxilla) including 204 basements(144 in the mandible and 60 in the maxilla). The patients were followed up for 3 years and recorded intact with computer. Analyse the information about patients' feelings and function and periodontal status of abutments. Results After 1 month, all patients were satisfied with esthetics and oral comfort, function significantly improved , no obvious effect on speaking. After 6 months, all dentures worked well. After treatment, the periodontal pocket decreased or disappeared [(4.0±0.5)mm contrast (1.1±0.5)mm], the tooth mobility improved. Analysing the information before and after treatment, all difference had statistical significance(P < 0.05). New gingival attachment formed in the teeth with bifurcation involvement;X-ray showed the lamina dura and periodontal ligament were clear, the height of alveolar bone remained stable, and the bone density increased. Only light gingivitis and gingival recession were found in few cases. All of the dentures succeeded, 100% of the abutment teeth survived, and no denture ceased function. Conclusion The telescopic denture is an effective method for dentition defect with periodontitis. It is also predictable, reliable, easy to realize and inexpensive and comfortable for patients.
