左布比卡因和布比卡因应用于下颌阻生第三磨牙拔除术的交叉实验研究

摘要: 目的 观察左布比卡因和布比卡因在下颌阻生第三磨牙拔除中的麻醉效果和术后镇痛效果。方法 选择2005—2007年在连云港市第一人民医院口腔科门诊就诊的需要拔除下颌阻生第三磨牙的患者(双侧第三磨牙阻生情况相近)20例,两侧分别使用左布比卡因(LB组)和布比卡因(B组)行神经阻滞麻醉,患者自身和手术医生分别对术中麻醉效果进行评价,患者自身评价采用数字评价法(NRS),手术医生评价采用面部表情评分法(FPS)。并对麻醉起效时间和麻醉持续时间进行比较,观察和记录麻醉并发症。结果 LB组NRS值为2.15±1.23,B组NRS值为2.10±1.55,两组差异无统计学意义(P > 0.05);LB组FPS值为1.95±1.10,B组FPS值为1.90±0.91,两组差异无统计学意义(P > 0.05);麻醉起效时间LB组为(3.64±0.50) min,B组为(3.73±0.44) min,两组差异无统计学意义(P > 0.05);术后麻醉持续时间LB组为(8.80±1.67) h, B组为(7.48±1.41) h,两组比较差异有统计学意义 (P < 0.05)。两组均无麻醉并发症发生。结论 左布比卡因应用于下颌阻生第三磨牙拔除安全有效,麻醉持续时间长,术后镇痛效果好。

Abstract: Comparison of levobupivacaine and bupivacaine in the extraction of impacted mandibular third molars: a cross-over trial . LI Xiang-xin. The First People’ Hospital of Lianyungang , Lianyungang 222002, China
Abstract: Objective To determine the duration and the effects of levobupivacaine and bupivacaine in the extraction of the impacted mandibular third molars. Methods Twenty patients with bilateral impacted mandible third molars were selected. Levobupivacaine and bupivacaine were injected into the two sides respectively. The rate of analgesia was judged by the patients and the operator. The onset time of analgesia and its duration and the postoperative side effects were observed and compared. Result Pain intensity was rated by the operator on a faces pain scale (FPS). The average FPS was 1.95±1.10 and 1.90±0.91 in group LB and B respectively (P > 0.05). The patients themselves evaluated their pain on a numerical rating scale (NRS). The average NRS was 2.15±1.23 and 2.10±1.55 in group LB and B respectively (P > 0.05). The average onset time was (3.64±0.50) min and (3.73±0.44) min respectively (P > 0.05). The duration was (8.80±1.67)h and (7.48±1.41)h respectively (P < 0.05). No side effects were observed in both groups. Conclusion Levobupivacaine is safe and effective in the extraction of the impacted mandibular third molars. It is a long-lasting local anesthetic with effective post-operative analgesia.
