复方甘菊利多卡因凝胶应用于下牙槽神经阻滞麻醉前表面麻醉的临床效果研究

摘要: 目的    评价复方甘菊利多卡因凝胶作为表面麻醉剂联合下牙槽神经阻滞麻醉(inferior alveolar nerve block,IANB)在口腔治疗中的应用效果。方法    选取2024年11月至2025年4月于西安医学院第三附属医院口腔病院口腔颌面外科就诊的需行IANB的青年患者120例,采用随机数字表法将所有患者分为试验组(复方甘菊利多卡因凝胶+ IANB)和对照组(同色凝胶牙膏+ IANB),每组各60例。比较2组患者麻醉前2 min(麻醉前)、麻醉进针时(麻醉时)、麻醉后2 min(麻醉后)的生理指标(血压、心率、脉率、血氧饱和度)和麻醉后的心理指标[疼痛视觉模拟量表(visual analogue scale,VAS)评分、Kuttner面部表情量表评分、Frankl治疗依从性量表评分、Houpt行为量表评分]差异。结果    试验组麻醉前、麻醉时、麻醉后的收缩压、舒张压、心率和脉率均呈现逐渐下降的趋势,对照组则呈现先升高后下降的趋势。而麻醉前、麻醉时、麻醉后试验组和对照组的血氧饱和度比较,差异均无统计学意义(均P > 0.05)。试验组VAS评分、Kuttner评分显著低于对照组,Frankl评分、Houpt评分显著高于对照组,差异均有统计学意义(均P < 0.05)。结论    复方甘菊利多卡因凝用于IANB前的表面麻醉能够有效减轻患者疼痛,降低治疗过程中的不适感,并提高治疗依从性。

Abstract: Objective    To evaluate the effectiveness of compound chamomile lidocaine gel as a surface anesthetic in conjunction with inferior alveolar nerve block(IANB)in dental procedures. Methods    From November 2024 to April 2025,120 patients received IANB at the Third Affiliated Hospital of Xi'an Medical University,and they were selected and randomly divided into the experimental group(compound chamomile lidocaine gel + IANB)and the control group(color-matched gel toothpaste + IANB),with 60 patients in each group. The changes in physiological indicators(blood pressure,heart rate,pulse rate,blood oxygen saturation)at 2 minutes before anesthesia(before anesthesia),during injection of anesthetics(during anesthesia),and at 2 minutes after anesthesia(after anesthesia)and psychological indicators after anesthesia(visual analogue scale score,Kuttner facial expression scale score,Frankl treatment compliance scale score,Houpt behavior scale score)were compared between the two groups. Results    In the experimental group,systolic blood pressure,diastolic blood pressure,heart rate,and pulse rate exhibited a gradually decreasing trend before,during,and after anesthesia,whereas the control group showed an initial increase followed by a decrease. In contrast,no statistically significant differences were observed in oxygen saturation between the experimental and control groups before,during,or after anesthesia(all P > 0.05). The VAS scores and Kuttner scores in the experimental group were significantly lower than those in the control group,while the Frankl scores and Houpt scores were significantly higher,with all differences being statistically significant(all P < 0.05). Conclusion    The application of compound chamomile lidocaine gel for surface anesthesia prior to IANB can effectively reduce pain,decrease discomfort during treatment,and improve treatment compliance.
