牙本质敏感

摘要: 提要:牙本质敏感是指暴露的牙本质对外界刺激产生短而尖锐的疼痛,并且不能归因于其特定原因引起的牙体缺损或病变,典型的刺激包括温度刺激、吹气刺激、机械性刺激或化学刺激。目前最为广泛接受的牙本质敏感机制是流体动力学理论。牙体硬组织的缺损和牙龈退缩所造成的牙本质暴露是牙本质敏感发生的前提。牙本质敏感的诊断需结合患者的病史、患者的主观感受、临床检查,属于排除性诊断。牙本质敏感在我国成人中比较普遍,患病率为29.7%。我国口腔医生对牙本质敏感的相关知识和临床诊断治疗等方面存在认知不足。预防牙本质敏感首先必须改变或去除危险因素。牙本质敏感的治疗原理包括减少牙本质小管内的液体流动和(或)阻断牙本质小管内的神经传导。抗敏感牙膏是首选推荐的、适合患者自己使用的一种牙本质敏感控制方法。

Abstract: Summary: Dentin hypersensitivity is defined as a short,sharp pain resulting from exposed dentin in response to stimuli,which are typically thermal,evaporative,tactile,osmotic or chemical and which cannot be ascribed to any other form of dental defect or disease. The most widely accepted mechanism of dentin hypersensitivity is the hydrodynamic theory. Two processes are essential for the development of dentin hypersensitivity:dentin must become exposed,through either loss of enamel or gingival recession,and the dentin tubules must be open to both the oral cavity and the pulp. To diagnose dentin hypersensitivity,clinicians must rely on patient-reported history and clinical examination. By definition,dentin hypersensitivity is a diagnosis of exclusion. Dentine hypersensitivity is common in Chinese adults,with the prevalence of 29.7%. On the other hand,most dentists in China have not grasped the knowledge,diagnosis and treatment of dentin hypersensitity. To prevent dentin hypersensitivity,predisposing factors and causes of dentin hypersensitivity should be removed or modified. The principle of treament relies on reducing fluid flow in the tubules,blocking the nerve response in the pulp or possibly both. Everyday use of desensitizing toothpastes should be considered and recommended as a noninvasive,inexpensive and efficacious first line of treatment.
