均角型骨性Ⅲ类错牙合拔牙与非拔牙治疗前后硬组织变化比较的研究

摘要:

目的 探求均角型骨性Ⅲ类错牙合的最适治疗手段。方法 辽宁省人民医院2000 — 2006年完成的18例均角型骨性Ⅲ类错牙合 患者中,拔牙组10例、未拔牙组8例,治疗前后拍头颅定位侧位片,进行描记、定点、测量,比较13项角度指标和2项线距指标。 结果 未拔牙组SNA、NA-PA、U1-NA、U1-SN角增大,U1-NA距增大,SNB、NP-FH角减小,ANB角增大至正常范围,与矫治前相比差异均有统计学意义(P均 < 0.05);拔牙组SNA、SNB、U1-NA、U1-SN角减小,ANB角增大至正常范围,与矫治前相比差异均有统计学意义(P < 0.05 或P < 0.01)。结论 均角型骨性Ⅲ类错牙合应优选非拔牙矫治以改善凹陷面型。

Abstract:Objective To investigate the hard tissue changes of skeletal Ⅲmalocclusion of average
GoGn-SN angles before and after treatment with extraction and non-extraction by cephalometric radiographs, and to identify the suitable therapeutic approach for patients with skeletal Ⅲ maloccsion at average MP-SN angles. Methods Ten cases treated by extraction and 8 cases by non-extraction of skeletalⅢ malocclusion at average GoGn-SN angles who completed their treatment processes were included in the study, and pre-trement and post-treatment cephalometric radiographs were performed to determine the changes.Thirteen angular measures and 2 linear measures were statistically analyzed. Results After treatment, the mean values of SNA, NA-PA, U1-SN, U1-NA angle and U1-NA distance in the non-extractive cases were significantly increased, whereas SNB and NP-FH were decreased dramatically, and the value of ANB was increased to the normal range; in contrast, the mean values of SNA, SNB, U1-SN, U1-NA angles were significantly decreased in extractive cases though the ANB was also increased significantly to the normal range. Conclusion Non-extraction is preferred to extraction in treating skeletal Ⅲ malocclusion of average GoGn-SN angle to improve the depressed profile.
