种植体周炎的手术治疗策略:2023年版《种植体周围疾病的预防和治疗——欧洲牙周病学会S3级临床实践指南》解读

摘要: 种植体周炎是种植修复后常见的炎症性并发症,其病理特征表现为种植体周软组织进行性炎症及骨吸收,严重影响种植体的长期存留率。为应对这一问题,欧洲牙周病学会(European Federation of Periodontology,EFP)于2023年6月发布了基于循证医学的种植体周围疾病预防与治疗S3级临床实践指南。该指南深入探讨了种植体周炎的手术治疗策略,涵盖了术前评估、术式选择、术后管理及疗效评价标准,为种植体周炎的手术治疗提供了系统、规范的路径。然而,目前临床实践中仍存在术式选择争议和疗效评价标准不统一等问题。文章结合最新研究证据,对该指南的临床要点进行整合与解读,旨在为种植体周炎的规范化治疗提供实践参考。

Abstract: Peri-implantitis,a prevalent inflammatory complication following dental implant rehabilitation,is pathologically defined by progressive inflammation of peri-implant soft tissues and bone resorption,severely compromising long-term implant survival. To address this issue,the European Federation of Periodontology(EFP)published the S3-level clinical practice guideline for the prevention and treatment of peri-implant diseases based on evidence-based medicine in June 2023. The guideline deeply explores the surgical treatment strategies for peri-implantitis,including preoperative evaluation,surgical modality selection,postoperative management,and efficacy assessment criteria,thereby offering a systematic and standardized pathway for surgical treatment of peri-implantitis. However,persistent controversies in surgical technique prioritization and heterogeneity in outcome evaluation metrics remain unresolved in contemporary practice. This review,drawing on the latest research findings,integrates and interpretes the clinical key points of the guideline,in order to offer a practical reference for the standardized treatment of peri-implantitis.
