三种酸蚀模式对不同复合树脂充填Ⅱ类洞微渗漏影响的体外研究

摘要: 目的    体外制备离体前磨牙Ⅱ类洞,研究酸蚀-冲洗、选择性酸蚀、自酸蚀3种酸蚀模式对5种复合树脂充填Ⅱ类洞微渗漏的影响。方法    选取2023年6—8月于中国医科大学附属口腔医院口腔颌面外科就诊患者因正畸治疗需要拔除的前磨牙135颗,制备Ⅱ类洞后根据酸蚀模式随机分为酸蚀-冲洗组、选择性酸蚀组、自酸蚀组,每组45颗。每组离体牙再随机分为5个亚组(每亚组9颗),分别使用3M FiltekTM Z350 XT通用纳米复合树脂(Z350亚组)及4种国产复合树脂[登泰克树脂(登泰克亚组)、安泰生物树脂(安泰生物亚组)、张江生物树脂(张江生物亚组)、正海生物树脂(正海生物亚组)]进行充填。冷热循环老化实验后采用染料渗入法在体视显微镜下观察评价各组微渗漏分级,扫描电镜下观察粘接界面特点,通过Micro-CT扫描计算银沉积物体积来评估微渗漏程度。结果    采用同种酸蚀模式的不同复合树脂充填Ⅱ类洞远中壁、龈壁微渗漏分级比较,差异均无统计学意义(均P > 0.05)。对于采用不同酸蚀模式的同种复合树脂充填Ⅱ类洞微渗漏分级比较,Z350亚组、登泰克亚组、安泰生物亚组和正海生物亚组远中壁微渗漏分级差异有统计学意义(均P < 0.05),龈壁微渗漏分级差异均无统计学意义(均P > 0.05);张江生物亚组远中壁和龈壁微渗漏分级差异均无统计学意义(均P > 0.05)。在各酸蚀模式组相同复合树脂亚组内,龈壁微渗漏分级均高于远中壁,差异均有统计学意义(均P < 0.001)。扫描电镜观察显示,酸蚀-冲洗组和选择性酸蚀组Ⅱ类洞远中壁和龈壁处树脂与牙体组织间结合较为紧密,粘接界面较平直完整;自酸蚀组树脂与牙体组织间裂隙宽长且不规整,粘接界面有破碎产生。Micro-CT分析结果显示,酸蚀模式、复合树脂类型均对银沉积物体积有影响,且二者存在交互效应(F值分别为57.204、501.046、3.093,均P < 0.05)。在同种酸蚀模式中,张江生物亚组银沉积物体积大于其他亚组;在同种复合树脂类型中,自酸蚀组银沉积物体积大于酸蚀-冲洗组和选择性酸蚀组,差异均有统计学意义(均P < 0.05)。结论    国产复合树脂(登泰克树脂、安泰生物树脂和正海生物树脂)充填Ⅱ类洞表现出与3M FiltekTM Z350 XT通用纳米复合树脂类似的边缘封闭性,且采用酸蚀-冲洗模式和选择性酸蚀模式相较于自酸蚀模式可降低其微渗漏程度。

Abstract: Objective    Class Ⅱ cavities are prepared in vitro in extracted premolars to investigate the effects of three acid etching modes,namely etch-and-rinse(EAR),selective-enamel etch(SEE),and self-etch(SE)on the microleakage of class Ⅱ cavity filled with five composite resins. Methods    A total of 135 premolars extracted from patients were selected,who visited the Department of Oral and Maxillofacial Surgery of the Affiliated Stomatological Hospital of China Medical University due to orthodontic treatment needs from June to August 2023. After preparing class Ⅱ cavities,they were randomly divided into three groups according to the acid etching mode:EAR group,SEE group,and SE group,with 45 teeth in each group. Each group was further randomly divided into five subgroups(9 teeth in each subgroup),and filled with 3M FiltekTM Z350 XT universal nanocomposite resin(Z350 subgroup)and four domestic composite resins [Dentex resin(Dentex subgroup),Antai biological resin(Antai biological subgroup),Zhangjiang biological resin(Zhangjiang biological subgroup),and Zhenghai biological resin(Zhenghai biological subgroup)]. After thermocycling,the microleakage grades of each group were evaluated under a stereomicroscope using the dye penetration method,and the characteristics of the bonding interface were observed under a scanning electron microscope(SEM). The degree of microleakage was evaluated by calculating the volume of silver deposits through micro-computed tomography(Micro-CT)scanning. Results    There was no statistically significant difference in the microleakage grades of the distal and gingival walls of class Ⅱ cavities filled with different composite resins using the same acid etching mode(P > 0.05). For the comparison of microleakage grades of class Ⅱ cavities filled with the same composite resin using different acid etching modes,there were statistically significant differences in the distal wall microleakage grades among the Z350 subgroup,Dentex subgroup,Antai biological subgroup,and Zhenghai biological subgroup(P < 0.05),while there were no statistically significant differences in the gingival wall microleakage grades(P > 0.05). There were no statistically significant differences in the distal and gingival wall microleakage grades of the Zhangjiang biological subgroup(all P > 0.05). Within the same composite resin subgroup of each acid etching mode group,the gingival wall microleakage grade was higher than that of the distal wall,and the differences were statistically significant(P < 0.001). Scanning electron microscopy observations showed that the bonding interface of the distal and gingival walls of class Ⅱ cavities in the EAR group and the SEE group was relatively tight and straight,while in the SE group,the micro-cracks of bonding interface was wide,long,and irregular,with fragmentation. Micro-CT analysis results indicated that both the acid etching mode and the type of composite resin affected the volume of silver deposition,and there was an interaction effect between them (F values were 57.204,501.046,and 3.093,all P < 0.05). Within the same acid etching mode,the volume of silver deposition in the Zhangjiang biological subgroup was greater than that in the other subgroups;within the same resin type,the volume of silver deposition in the SE group was greater than that in the EAR group and the SEE group,and the differences were statistically significant(P < 0.05). Conclusion    Domestic composite resins(Dentex resin,Antai biological resin,and Zhenghai biological resin)show similar marginal sealing properties to 3M FiltekTM Z350 XT universal nanocomposite resin when they are used for filling class Ⅱ cavities. The EAR mode and the SEE mode can reduce the microleakage degree compared with the SE mode.
