生物陶瓷类根管封闭剂结合单尖充填法与热牙胶垂直加压充填法临床效果比较的Meta分析

摘要： 目的    比较生物陶瓷类根管封闭剂结合单尖充填法与热牙胶垂直加压充填法的临床效果。方法    计算机检索Pubmed、Cochrane Library、Web of Sicience、CNKI、Wanfang、CBM数据库，检索时间为自建库至2021-11-08。查找生物陶瓷类根管封闭剂结合单尖充填法（研究组）和热牙胶垂直加压充填法（对照组）临床效果相关文献，通过Meta分析比较两组治疗有效率、疼痛情况、根管充填质量及充填时间。结果    共纳入9篇相关文献，其中6篇为随机对照研究，3篇为队列研究。Meta分析结果显示，在随机对照研究中，研究组治疗有效率高于对照组，但差异无统计学意义（OR = 0.88，95%CI：0.40 ~ 1.96，P > 0.05）。在队列研究中，对照组治疗有效率高于研究组，但差异也无统计学意义（OR = 1.11，95%CI：0.49 ~ 2.52，P > 0.05）。两组根管充填质量比较，差异无统计学意义（OR = 1.80，95%CI：0.57 ~ 5.73，P > 0.05）。研究组根管充填时间短于对照组，且治疗后短期内研究组的疼痛程度较对照组轻。结论    生物陶瓷类根管封闭剂结合单尖充填法与热牙胶垂直加压法一样可达到良好的临床效果，而且根管充填时间更短，患者舒适度较好，建议临床推广应用。

Abstract: Objective    To compare the clinical effect of bioceramic sealer combined with single cone obturation technique with warm vertical condensation. Methods    Pubmed，Cochrane Library，Web of Sicience，CNKI，Wanfang and CBM were searched by computer （the time was from the establishment of the database to November 8，2021）. Search for the related literatures on the therapeutic effect of bioceramic combined with single cone obturation technique（study group）and warm vertical condensation （control group），and compare the clinical efficiency，postoperative pain，filling quality and filling time of the two groups of root filling methods by Meta-analysis. Results    Nine studies were included，including 6 randomized controlled clinical studies and 3 cohort studies. The results of Meta-analysis showed that in the randomized controlled trial，the effective rate of the study group was higher than that of the control group，but the difference was not statistically significant（OR = 0.88，95%CI：0.40-1.96，P > 0.05） . In the cohort study，the effective rate of the control group was higher than that of the study group，but there was no significant difference（OR = 1.11，95%CI：0.49-2.52，P > 0.05）. There was no significant difference in the quality of root canal filling between the two groups（OR = 1.80，95%CI：0.57-5.73，P > 0.05）. The time of root canal filling in the study group was shorter than that in the control group，and the degree of pain in the study group was less than that in the control group in the short term after treatment. Conclusion    The bioceramic sealer combined with single cone obturation technique can achieve the same good clinical efficacy as the warm vertical condensation method. Moreover，the operation time is shorter，and the patient feels more comfortable，which is worthy of clinical popularization and application.
