树脂渗透技术治疗早期龋的临床观察研究

摘要: 目的 观察树脂渗透技术治疗前牙唇面早期龋的临床效果及稳定性。方法 根据纳入与排除标准,对10例患者的27颗前牙唇面早期龋白垩斑进行树脂渗透治疗。临床治疗效果分为3级:有效、部分有效、无效。患者于术前及术后即刻、6个月、12个月进行检查,均在同一标准下拍摄口内像,采用Image J测量术前、术后即刻白垩斑面积(the area of white spot,W)及患牙唇面面积(the area of labial surface,L),计算W/L的值,即白垩斑面积所占比。采用两样本非参数Wilcoxon秩和检验进行统计学分析,P < 0.05为差异具有统计学意义。结果 27颗患牙的临床治疗效果:8颗(29.6%)为有效,19颗(70.4%)为部分有效,0颗为无效。部分有效的患牙术前W/L平均值是28.71%,术后即刻降至4.34%,经两样本非参数Wilcoxon秩和检验,P < 0.05。术后6、12个月复查,通过视诊和探诊未见有龋洞形成,未见牙面变色与着色。结论 树脂渗透技术治疗早期龋的临床效果确切,在12个月观察期内效果保持稳定,未见继续脱矿发展形成龋洞,可以作为早期龋微创治疗的新选择。

Abstract: Objective To observe the clinical effects and color stability of early caries lesions treated with resin infiltration. Methods According to the inclusion/ exclusion criteria,and there were 10 patients (27 teeth) with early caries lesions were treated with resin infiltration. The results were classified into three levels:effective,partially effective,and unchanged. Standardized digital photographs of the patients were taken before treatment,and immediately,6 and 12 months after treatment,respectively. Pictures were analyzed using the Image J software. The area of white spot (W) and whole tooth labial surface (L) were measured,and then we analyzed the ratio of W/L. A nonparametric Wilcoxon rank test was used to do the statistical analysis,and P < 0.05 was considered statistically significant. Results Among the 27 teeth,8 teeth (29.6%) were evaluated as effective,19 teeth (70.4%) were evaluated as partial effective,and no tooth was evaluated unchanged. The mean value of the W/L decreased from 28.71% to 4.34% after resin infiltration treatment (by nonparametric Wilcoxon rank test,P < 0.05). After the 6 and 12 months follow-up,we didn’t observe the color change or the caries enlarged through the examination. Conclusion The clinical effect of early caries lesions treated with resin infiltration is significant. During 12 months of follow-up,the clinical effect keeps stable and no further demineralization is observed. Therefore,resin infiltration can be used as a novel choice for minimally invasive treatment for early caries lesions.
