    Management of cervical cancer during pregnancy:current situation and the development.

    Cervical cancer is one of the most common gynecological malignancies during pregnancy.The management of cervical cancer during pregnancy depends on the following factors:tumor stage,histological type,gestational age,state of lymph nodes and desire regarding fertility.The treatment for patients who want to preserve pregnancy needs to be individualized and requires a multidisciplinary team.This article reviews the researches about management of cervical cancer during pregnancy.

    The coverage rate of HPV vaccination in China is still very low,and the main reason may be related to people's confusion about the choice of vaccine types,the best age of vaccination,vaccination procedures and prices and other factors.This paper briefly describes the etiology of cervical cancer,the approval of HPV vaccine,the application status of HPV vaccine in foreign countries,and the current situation and challenges in the use of cervical cancer vaccine in China,and advocates that obstetrics and gynaecology experts/disease control experts actively promote the prevention and control of cervical cancer,so that young girls and their parents understand the knowledge related to cervical cancer and HPV vaccine,and young girls can be vaccinated with HPV vaccine as early as possible,so as to improve the coverage rate of HPV vaccination and eliminate cervical cancer.

    As the treatment methods develop,the prognosis of cervical cancer has been significantly improved.Surgery and radiotherapy are still the main treatments for cervical cancer,accompanied by chemotherapy.For young patients with cervical cancer,radiotherapy can result in the ovarian failure and lead to a series of menopausal symptoms.Radiotherapy can also cause a lot of other complications and affect the quality of life.Chemotherapy,especially adjuvant chemotherapy before and after surgery,is drawing more and more attention recently.Therefore we made a summary of the latest progress in the strategies for postoperative adjuvant chemotherapy and preoperative neoadjuvant chemotherapy for cervical cancer.

    The treatment for locally advanced cervical cancer,the surgical path of cervical cancer and the surgical indications for preserving fertility are closely related to the stage of cervical cancer,which still have great controversies.With the release of the new staging of cervical cancer by the International Union of Obstetricians and Gynecologists(FIGO)in 2018,relevant clinical diagnosis and treatment should adapt to the new staging.This paper introduces the above problems by referring to relevant researches at home and abroad.

    Perineural invasion in early-stage cervical cancer,is a new risk factor for the prognosis of cervical cancer.There may be a certain potential risk in retaining nerves and this is the reason why nerve-sparing radical hysterectomy(NSRH) is an important hot topic in clinical research at present.The aim to study PNI in cervical cancer is to develop clear indications for NSRH,in order to show the advantage of this surgical technique.PNI is one of the main factors affecting NSRH indications.Therefore,it is necessary to clarify the indications for NSRH,and it is imperative.

    Radical hysterectomy(RH)with bilateral pelvic lymph node dissection is recommended as the standard treatment for early-stage cervical cancer(FIGO stage ⅠB1 or ⅡA1 disease).Minimally invasive surgery(MIS)has been adopted as an optimal treatment for early cervical cancer since 20th century for its advantages in perioperative outcomes and comparable oncological outcomes.Not until 2018 were two articles from MD Anderson Center published in The New England Journal of Medicine reversed the situation.Their results suggested that patients who underwent MIS for RH had higher recurrence rates and worse overall survival compared to patients that underwent open surgery.Since then,controversy about open surgery and MIS has continued.This article will discuss the current controversy and stragegies of laparoscopic surgery for cervical cancer.
