    Relationship between brain growths with werum epidermal growth factor level in preterm infants.

    Objective  〖WTBZTo investigate the association of brain growth with serum epidermal growth factor level in preterm infants.

    WTHZMethods  〖WTBZThere are 30 cases of appropriate for gestational age preterm infants who continuous admitted in our newborn nursery within 24 hours after birth from August 2005 to April enrolled. Enzymelinked immunosorbent assay (ELISA) was used to determine the serum level of EGF within 24 hours (t0) and on the 8th day (t1) after birth.Calculate the difference from t0 with t1 and the portrait change velocity (t).NBNA have been operated by a trained neonatal specialty nursing staff,when the infant is 6 weeks after birth and corrected 1 month age.Perform the correlation analysis of these laboratory data and NBNA score,head circumference growth after birth.If P<005,it means statistics significance of difference.

    WTHZResults  〖WTBZThe laboratory result showed that t0 is (23471±5807)pg/mL,t1 is (34781±8749)pg/mL,and t is (1214±185)pg/(mL·d).Pearson correlation analysis showed that NBNA scores,head circumference increase of 6 weeks after birth and corrected 1 month age were associated with t1 and t (P<005),but not t0 (P>005).

    WTHZConclusion  〖WTBZThe presented data indicate the serum EGF concentration has been upgrading in preterm infants after birth,hint that increasing synthesis and release of EGF in vivo may promote organ maturity and tissue repair.It is more important that higher serum level or quicker upstroke velocity of EGF in the first week after birth would be associated with faster head growth and brain development,prognosticating lower incidence of psychoneural sequelae,and more optimistic long term outcome in preterm infants.

    Expression of neutrophil CD64 and CD11b in diagnosis of neonatal septicemia

    Objective  〖WTBZTo evaluate the diagnostic utilities of two neutrophil antigens(CD11b and CD64) for early identification of neonatal sepsis related to bacterial infection.

    WTHZMethods  〖WTBZThe group of sepsis consisted of 36 newborns and the control group included 26 healthy newborns.The blood samples were obtain just into hospital and recovery stage in the group of sepsis and were collected only once in the control.In sepsis group,samples were also taken for bacterial culture before treatment.CD64 and CD11b were quantified with the whole blood flow cytometry and direct immunofluorescence technique.We also mearured Creactive protein(CRP).The sensitivity,specificity,positive and negative predictive values,Youdens index for CD64,CD11b and CRP were calculated.These values were compared with eath other.

    WTHZResults  〖WTBZThe expression of CD64 and CD11b in neonates with sepsis were significantly higher than that in the group of control.These markers are considered positive if CD64≥35MFICD11b≥1300MFICRP8mg/L.CD64 has the highest sensitivity(957%),specificity(958%),positive predictive(956%),negative predictive(954%),and Youdens index(915%).

    WTHZConclusion  〖WTBZCD64 and CD11b are the credible marker in the early diagnosis of neonatal sepsis,and they may play important role in the treatment of neonatal sepsis.

    Analysis of feeding methode and gastrointestinal injury neonates with critically ill.

    Objective  〖WTBZTo investigate the influence of the early feeding on the gastrointestinal function of the critically ill neonates and the relationship between the start point of feeding time and the occurrence of gastrointestinal injury.

    WTHZMethods  〖WTBZThe clinical information of 56 neonates in our department was analyzed.All neonates started to be fed 6 hours later after birth following different ways of feeding: either feeding in microdosis (01~4mL/kg·d) or feeding according to the required volume (start from 10~20mL/kg·d then added 10~15mL/kg per time per day),The incidence of the gastrointestinal damage was compared with two feeding methods.

    WTHZResults  〖WTBZ(1)The incidence of the gastrointestinal damage was significantly lower in the neonates of microdosis group than the required feeding group(P<005)(2) The incidence of the gastrointestinal damage was significantly higher in the premature infant group than the nomal term infant group(P<005).(3)The incidence of gastrointestinal damage of 24th hour after birth group was higher than 48th hour after birth group.(P<005).(4)The incidence was significantly higher in the seriously critically ill neonates than the critically ill ones.

    WTHZConclusion  〖WTBZTo reduce the incidence of the gastrointestinal damage,the critically ill neonates should not be fed according to the required volume early after birth, especially in the first 24 hours of birth.It  will suggested that the feeding should be started to use a microdosis manner using 1∶〖KG-*21 diluted mild at the first 6 hours followed by a slowly manner to increase the volume till the whole milk after 48 hours.

    Objective  〖WTBZTo explore the relation of HOCO system to cause of SGA,searching for a new way in etiology prophylaxis and treatment for it.

    WTHZMethods  〖WTBZ50 full term SGA neonates (nosymmetry) in our hospital and obstetric hospital were chosen as objects,that were grouped into two,one named SGA1 was 30 whose mother without pregnancy induced hypertation (PIH),the other named SGA2 was those whose mother with middle or severe PIH.Meanwhile,a group of 20 healthy full term AGA neonates named AGA were chosen as reference.The umbilical artery blood specimens were collected immediately after the newborns were delivered,umbilical cord specimens near placenta were kept as well.HO1 activity in cord blood was measured by Dual Wavelength Spectrophotometer.HbCO percentage in plasma of cord blood was measured by Chalmers hemoglobin method.The expression of HO1 in cord blood vessels was detected by the method of immunohistochemistry.

    WTHZResults  〖WTBZ1.The comparisons of HO1 activity and HbCO percentage in the cord blood of different groups of neonates:SGA1 and SGA2 are significantly lower than AGA,shown on statistics with significant difference (P<001).No significant difference shows between SGA1 and SGA2 (P>005).2.HO1activity in cord blood of AGA,SGA1 and SGA2 all shows positive correlation with birth weight (P<005).3 Expression of HO1 in cord blood vessels:The intensity of HO1 expression of both SGA1 and SGA2 are higher than AGA,which shows significant difference (P<001).No significant difference is found between SGA1 and SGA2 (P>005).

    WTHZConclusion  〖WTBZHO1 activity in cord blood correlates closely with intrauterine growth and nutrition of the newborn.

    Curative effect of cattle encephalon glycoside and ignotin injection on neonatal hypoxicischemic encephalopathy

    Abstract ( )  

    Objective  To observe curative effect of cattle encephalon glycoside and ignotin injection (GM1 made in China) on neonatal hypoxicischemic encephalopathy(HIE).Methods  70 cases neonates with HIE were randomly divided into two groupsGM1 treated group (n=36) and nicholin or fructose1,6diphosphate(FDP) treated control group(n=34).The recover time of abnormal neurological symptoms and signs and the score of neonatal behavioral neurological assessment(NBNA) was evaluated in acute phase of HIE.Results  The recover time of abnormal neurological symptoms and signs and the score of NBNA there wasn't significant difference between GM1 treated group and control group (P>0.05).Conclusion  GM1 administ ration has early benefit to the patients with HIE.

    Studies on the relationship between astrocyte and clinical appearance in neonatal hypoxicischemic encephalopathy

    Objective  To observe the relationship between the change of astrocyte and clinical appearance in neonatal hypoxicischemia encephalopathy (HIE).Methods  The pathological brain specimens of 25 newborn who died of HIE were observed macroscopically and microscopically.The immunochemistry staining with GFAP was used to observe the change of astrocyte.The relationship between astrocyte and the occurrence of HIE,course of disease and newborn degree of maturity were further synthetically analysed.Results  The brain specimens of 25 newborn HIE have differentdegreed astrocyte hyperplasia,Cerebellar hyperplasia show that hyperplasia is severe in the cases who dead within 24 hours(P<0.05),the longer survived time the less hyperplasia it showed.Cerebral and medulla oblongata astrocyte hyperplasia is independent of survival time.Cerebral astrocyte hyperplasia in the chronic incomplete hypoxia cases are severe(P<0.05); the change of medulla oblongata in the acute complete hypoxia cases are severe(P<0.05).the change of cerebral cortex and medulla oblongata are all severe in the cases of mixed hypoxia(P<0.05).Conclusion  The degree of astrocyte hyperplasi are closely related to the occurrence and course of disease in HIE.

    Study on the relationship between ghrelin level in cord blood of small for gestational age with the intrauterine fetal growth and metabolism axis.

    Objective  Objective of this study is to compare the Ghrelin concentration in the umbilical cord blood of SGA and normal control group,and analyze the relationship between Ghrelin concentration in the umbilical cord ghrelin with the following indexes: birth weight,birth length,head circumference,BMI placenta weight and placenta volume,and with IGF1,insulin,insulin sensitivity index and glucose.By approaching the interrelationship and effect between the Ghrelin concentration in the umbilical cord  with fetal growth and metabolism axis,especially the relationship with IGF1.Methods  78 neonates were enrolled in this study.There were 22 cases of SGA were enrolled in analyzed,42 case of full term AGA were enrolled in normal control group.The concentration of Ghrelin,Insulin and IGF1 was measured by ELISA assays.The birth weight and length,head circumference,the weight of placenta and the volume of placenta were measured.BMI and ISI were then calculated out.Results  1.Ghrelin concentration in the umbilical cord blood of SGA was significantly higher than that detected in normal control group (P<0.05).2.Ghrelin concentration in the umbilical cord blood of neonates is negatively correlated with birth weight,birth length,head circumference,BMI placenta weight and placenta volume,There are statistical significance(P<0.05).3.Ghrelin concentration in the umbilical cord blood of neonates is negatively correlated with IGF1,insulin,ISI,There are statistical significance(P<0.05).It is not correlated with glucose(r=0.081,P=0.454).4.According partial correlation analysis,controlling  birth weight,birth length,placenta weight,BMI and insulin,Ghrelin concentration in the umbilical cord blood of neonates is negatively correlated with IGF15.There was not significant difference in Ghrelin level between cesareandelivered and spontaneousdelivered neonates(P=0.956).so as the Ghrelin level between male and female neonates (P=0.385).Conclusion  Ghrelin can reflect the intrauterine fetal development and nutrition statue and  take part in the regulation of intrauterine fetal growth and metabolism by negative feedback.The insulin resistance maybe exists in SGA.The high concentration of Ghrelin did not stimulate the release of insulin also indicated the possibility of Ghrelin resistance of GHSR or conduction path of postamble receptor.

    Investigation on the respiratory syncytial virus of acute lower respiratory tract infection in Kunming children's hospital.

    Objective  To study the infections state of respiratory syncytial  virus(RSV) in children suffered from acute lower respiratory tract infection from April 2006 to April 2007 in Kunming area.Methods  Children suffering from acute lower respiratory tract infection(ALRI) who were admitted to the Kunming Children's Hospital from April 2006 to April 2007 were chosen.Their nasopharyngeal secretions(NPS) were obtained for identification of RSV antigen by indirect immunofluorescence assay.Results  Among the all 1112 samples,276(2482%) positive results were determined.The ratio of male to female was 221∶〖KG-*21.RSV all can be detected in a year,the peak seasons of RSV infection were from October to December.Most of RSV infection cases were less than 3 years old.Conclusion  RSV is the most common viral pathogen in children hospitalized with acute respiratory infection in Kunming area

    Analysis of endothelinB receptor gene in Chinese patients with sporadic congenital megacolon.

    Objective  To probe the characteristics of mutations and polymorphisms of EDNRB gene in Chinese patients with sporadic Hirschsprung's(sHD) disease,and analyze the differences between different ethnics with sHD.

    Methods  Blood from 92 cases with sHD,parents of 32 cases(32/92) and 60 normal children as control was collected.Genomic DNA was extracted from peripheral blood and single strand conformation polymorphisms analysis of polymerase chain reaction products(PCRSSCP) was used to analyze exon2 of EDNRB gene in all cases.DNA sequencing was used to detect the change of nucleotide in positive cases and compare the characteristics of mutations and polymorphisms with other ethnics.Results  No mutations and polymorphic sites were detected in exon2 of EDNRB gene in all cases with sHD.Conclusion  Patients with sHD in Chinese mainland share the same characteristics of no mutations and polymorphisms in exon2 of EDNRB gene with many ethnics in earth,which is different from that in South Africa and HongKong.
