Abstract:

Objective　To investigate the association between tooth loss and hypertension in older adults by cross-sec tional investigation. Methods　This study included 3677 community-dwelling older adults aged 50 and above from the Shanghai Aging Study. Number of teeth lost was obtained from self-reporting questionnaire. Participants were diagnosed as ‘hypertension’ and ‘non-hypertension’ by their blood pressure using 2017 AHA guideline. Multivariate logistic regression model was applied to examine the association between tooth loss and hypertension. Results　The participants had an average of 9.67 teeth lost. Individuals with hypertension lost 10.88 teeth on average，significantly higher than those without hypertension （8.95） （P < 0.001）. After adjusted for age，BMI，cigarette smoking，heart disease，diabetes and overweight，tooth loss between 4 and 14 was significantly associated with grade I hypertension （OR = 1.22，95%CI：1.02～1.45），tooth loss over 15 was significantly associated with grade III hypertension （OR = 1.55，95%CI：1.09～2.20）. Conclusion　Too much tooth loss is associated with hypertension. Poor oral health might be considered as a risk factor of high blood pressure among older Chinese population.

摘要：

目的　采用横断面调查的方法研究中老年人缺牙数目与高血压的相关性。方法　选取2010—2012年上海老年队列研究中50岁以上社区中老年人共3677名，收集他们的血压值和缺失牙数目。缺失牙数目采用自我报告调查问卷收集。根据2017美国高血压指南的划分标准，将样本人群划分为高血压组和非高血压组，采用多元logistic回归分析高血压和缺牙数目间的关系。结果　样本人群平均缺牙9.67颗。高血压组人均缺牙10.88颗，非高血压组人均缺牙8.95颗（P < 0.001）。调整年龄、BMI、吸烟史、心脏病史、糖尿病史和是否超重后，缺牙4～14颗与Ⅰ级高血压显著相关（OR = 1.22，95%CI：1.02～1.45），缺牙≥15颗与Ⅲ级高血压显著相关（OR = 1.55，95%CI：1.09～2.20）。结论　缺失牙过多与高血压显著相关。不良口腔健康状况可能是中国中老年人高血压的危险因素。
