安氏Ⅱ类1分类与安氏Ⅰ类错牙合畸形患者牙弓宽度比较的Meta分析

摘要：

目的    对比较安氏Ⅱ类1分类与安氏Ⅰ类错牙合畸形患者牙弓宽度的文献进行系统评价，探讨安氏Ⅱ类1分类错牙合畸形患者牙弓宽度的特点。方法    计算机检索 PubMed、Embase、Cochrane数据库、中国知网（CNKI），收集国内外所有已发表的比较安氏Ⅱ类1分类与安氏Ⅰ类错牙合畸形牙弓宽度的中英文相关文献，检索时间为 2000-01-01至 2018-03-30，对文献语言没有限制。由2名研究员根据纳入和排除标准进行文献筛选，之后采用 RevMan 5.3软件进行Meta分析。结果    初步检索后得到955篇文献，经筛选后最终纳入8篇文献进行分析。Meta分析结果显示，安氏Ⅱ类1分类错牙合畸形患者的上颌前、中、后段宽度均小于安氏Ⅰ类患者，差异有统计学意义（P < 0.05）；而两类错牙合畸形患者的下颌前、中、后段牙弓宽度差异均无统计学意义（P > 0.05）。结论    安氏Ⅱ类1分类上颌牙弓宽度小于安氏Ⅰ类，而下颌牙弓宽度与安氏Ⅰ类相比无差异。因此，安氏Ⅱ类1分类患者在正畸治疗过程中应考虑上颌扩弓治疗，注意上下颌牙列弓形的匹配。

Abstract:

Objective　To systematically evaluate the literature comparing dental arch width between Class Ⅱ Division 1 and Class Ⅰ malocclusion patients and investigate the characteristics of dental arch width of Class Ⅱ Division 1 malocclusion. Methods　The PubMed，Embase，Cochrane database and CNKI were searched to collect all published Chinese and English related literature which compared dental arch width between Class Ⅱ Division 1 and Class Ⅰ malocclusion patients. The retrieval time was from January 1，2000 to March 30，2018. There was no limit on the language of literature. Two researchers screened the  literature according to the inclusive and exclusive criteria. And the data analysis was made by RevMan5.3. Results　Totally 955 articles were obtained after initial searching，and finally 8 articles were selected for the meta analysis. The meta analysis results showed that the width of the maxillary anterior，middle and posterior segments of the Class Ⅱ Division 1 was smaller than that of Class Ⅰ，and the difference was statistically significant （P < 0.05）. There was no significant difference in the width of the anterior，middle and posterior segments of mandibular arch between the two malocclusions （P > 0.05）. Conclusion　The maxillary arch width of Class Ⅱ Division 1 was smaller than that of Class Ⅰ，while the width of mandibular arch was similar. Therefore，during the orthodontic treatment，maxillary expansion and the matching of the maxillary and mandibular arches should be considered in Class Ⅱ Division 1 patients.
