全解剖式氧化锆全瓷冠用于固定义齿美学修复临床评价

摘要:

目的    通过比较两种氧化锆全瓷冠的临床美学效果,探讨全解剖式氧化锆全瓷冠的临床应用前景。方法        2015年11月至2016年4月于中国医科大学附属口腔医院修复二科行前牙及前磨牙固定义齿美学修复患者87例,随机分为两组,研究组42例应用TRIOS口内扫描仪获取三维数字模型,切削制作“TT”玉瓷全解剖式氧化锆全冠;对照组45例硅橡胶取模,常规制作“HT”瓷倍健有饰瓷氧化锆全瓷冠。修复完成后,由患者本人从5个方面对修复体满意度进行评分。3名修复科医生根据由Ryge评价标准改编的评分系统对两组修复体的美观性进行评分。修复体完成后按照改良公共卫生署(USPHS)标准对两组修复体进行随访评价。结果    研究组患者对修复体的满意度在颜色分布、个体特征方面低于对照组 (P<0.05)。临床医生对修复体美观性评分,研究组为(1.838 ± 0.169)分,与对照组[(1.631 ± 0.149)分]比较差异无统计学意义(P>0.05)。修复体随访结果显示,研究组在边缘适合性、牙龈反应、修复体完整性方面优于对照组(P<0.05)。结论    全解剖式氧化锆全瓷冠在前牙及前磨牙美学修复中显示出较高的美学特性,具有较好的临床应用前景。

Abstract:

Objective    To study the clinical application prospect of the monolithic zirconia crown by means of comparing clinical esthetic effects of two kinds of zirconia crowns. Methods    This survey involved 87 individuals receiving anterior and premolar teeth esthetic fixed dental prostheses treatment. Totally 42 participants were allocated to experimental group which used TRIOS intraoral scanner to receive a 3D digital model of corresponding positions,and then “TT”zirconia was profiled to produce monolithic zirconia crowns. Totally 45 participants were allocated to control group,in which silicon rubber impression was used to produce traditional veneered zirconia crowns. The satisfaction degree of these patients with their restorations was scored five aspects. Three prosthodontists assessed the esthetic appearance of both groups through the scoring system modified by Ryge standard. After the treatment,the clinical effects were assessed by the same prosthodontist through the modified USPHS standard. Results    In the satisfaction survey,patients in experimental group achieved lower satisfaction degree than in control group in the aspect of color distribution and individual characteristics(P<0.05). The assessment of the esthetic property of three prosthodontists showed no significant difference(P>0.05)between experimental group(1.838 ± 0.169)and control group(1.631 ± 0.149). According to the follow-up evaluation,experimental group showed great advantage over control group in the aspect of marginal adaptation,gingival reaction and integrity of restorations(P < 0.05). Conclusion    Monolithic zirconia crowns display excellent esthetic property in anterior and premolar teeth,and achieve great clinical application prospect.
