喷砂洁治在正畸固定矫治期间临床应用研究

摘要:

目的    观察甘氨酸喷砂和碳酸氢钠喷砂对佩戴正畸固定矫治器患者口腔卫生和牙周状况的影响。方法    选取2013年8月至2014年8月于中国医科大学附属口腔医院牙周科就诊,正畸期间出现菌斑性龈炎症状的固定矫治患者33例,共648颗牙,进行左右半口自身对照实验,左右半口内各324颗牙,随机分成实验组和对照组,实验组用甘氨酸喷砂治疗,对照组用碳酸氢钠喷砂治疗。在治疗前以及治疗后1周检查牙周探诊深度(PD)、牙龈指数(GI)、龈沟出血指数(BI)、菌斑指数(PLI),记录治疗所需时间、治疗后即刻牙龈出血牙数和患者的舒适度。结果    实验组治疗前后BI、GI、PLI差异均有统计学意义(均P<0.05);对照组治疗前后BI、GI、PLI差异均有统计学意义(均P<0.05);两组之间舒适度和即刻牙龈出血牙数差异亦均有统计学意义(均P<0.05)。结论    两种喷砂治疗均能有效改善固定正畸患者牙周状况,但甘氨酸喷砂舒适度更好、在治疗过程中对牙龈几乎无损伤。

Abstract:

Objective    To observe the effectiveness of glycine powder air-polishing and sodium bicarbonate air-polishing with orthodontic appliances on periodontal health. Methods    The trial was conducted as a split-mouth designed study including 33 patients who with dental plaque-induced gingivial disease needed fixed orthodontics were randomly divided into two groups:the treatment group(glycine powder air-polishing)and the control group(sodium bicarbonate air-polishing). The patients were reexamined at baseline,one week after treatment,and the following parameters were recorded:probing pocket depth(PD),bleeding index(BI),gingival index(GI),plaque index(PLI). The time needed for treatment,the number of teeth with gum immediately bleeding after treatment and the patients’ satisfaction levels. Results    The treatment group resulted in significant reductions of BI(P < 0.05), GI(P < 0.05)and PLI(P < 0.05). The control group resulted in significant reductions of BI(P < 0.05),GI(P < 0.05)and PLI(P < 0.05). The differences in the number of teeth with gum immediately bleeding after treatment(P < 0.05)and the satisfaction investigation(P < 0.05)between the two groups were statistically significant. Conclusion    Two methods can effectively improve periodontal health,but glycine powder air polishing offers greater patient satisfaction and almost on damage to the gums during the course of therapy.
