CarisolvⅢ化学机械去龋法对Ⅱ类洞微渗漏影响研究

摘要:

目的    探讨化学机械去龋制剂CarisolvⅢ对Ⅱ类洞光固化复合树脂充填术后微渗漏的影响。方法    本研究于2014年6月在中国医科大学附属口腔医院中心实验室和东北大学研究所完成。收集中国医科大学附属口腔医院口腔颌面外科门诊因各种原因拔除的恒磨牙30例,要求近中、远中均有龋坏,两侧龋坏大小、深度、范围相近。将样本的近中洞、远中洞随机分为实验组和对照组:实验组采用CarisolvⅢ化学机械法去龋;对照组采用传统慢速球钻去龋。所有样本光固化复合树脂充填后,经500次温差循环实验,置于0.5%碱性品红溶液中浸泡24 h,沿牙体长轴近远中向纵剖,在体视显微镜下对染料渗入情况进行等级评分(0 ~ 3分),在扫描电镜下观察充填体与牙体间微缝隙的宽度以评价微渗漏情况。染料渗入法评分采用Manu-Whiney U检验进行分析。微缝隙宽度采用两个样本t检验进行分析。结果    与对照组比较,实验组轴壁、龈壁染料渗入评分差异有统计学意义(P<0.05);各组内轴壁微渗漏明显少于龈壁(P<0.05)。扫描电镜下微缝隙宽度测量结果表明,组间轴壁、龈壁微缝隙宽度差异有统计学意义(P<0.05);组内轴壁微缝隙宽度明显小于龈壁(P<0.05)。结论    CarisolvⅢ能有效减少Ⅱ类洞光固化复合树脂充填术后微渗漏的发生;与龈壁相比,轴壁微渗漏更小。

Abstract:

Objective    To evaluate the effect of Carisolv Ⅲ as chemo-mechanical caries removal agent on microleakage of class Ⅱ. Methods    Thirty extracted permanent molars with same caries in both mesial surface and distal surface were collected.One cavity was prepared by CarisolvⅢ(the experimental group),and the other cavity was prepared by low-speed bur(the control group). All samples were filled with composite resin. After 500 thermocycling,all samples were stained with 0.5% fuchsin solution for 24 hours,and teeth were mesial distal cut along the tooth long axis, in accordance with the stereomicroscope dye penetration level score(0 ~ 3)and scanning electron microscopy to observe the microleakage. Dyes penetrate score results were analyzed by Manu-Whiney U test;micro-crack width results by SEM were analyzed by t-test using the SPSS 10.0 software package. Test the effectiveness of α of 0.05. Results    There were significant differences in Dyes penetrate score between two groups(P<0.05). Microleakage of axial walls was significantly less than gingival walls(P<0.05). There were significant differences in micro-crack width of axial walls and gingival wall by SEM between two groups(P<0.05). Micro-crack width of axial walls was significantly less than gingival walls(P<0.05). Conclusion    Chemo-mechanical caries removal with CarisolvⅢ can effectively reduce the microleakage of class Ⅱ. Microleakage of axial walls is less than gingival walls.
