寻常型天疱疮患者血清抗桥粒芯蛋白3抗体水平及其影响因素分析

摘要: 目的    研究寻常型天疱疮患者血清抗桥粒芯蛋白3(desmoglein 3,Dsg3)抗体水平及其影响因素。方法    选取2017年6月至2020年12月于南京市口腔医院口腔黏膜病科就诊的寻常型天疱疮患者30例,检查患者口腔病损部位,评估口腔损害严重程度,并采集患者血清样本。采用ELISA检测血清样本中与致病相关的抗Dsg3抗体水平,并分析血清抗Dsg3抗体水平的相关影响因素。结果    寻常型天疱疮患者血清中抗Dsg3抗体水平为(116.52 ± 7.35)U/mL。抗Dsg3抗体水平与寻常型天疱疮患者年龄、性别及口腔病损部位均无关(均P > 0.05),与口腔损害严重程度有关(Z = -3.070,P = 0.002)。结论    血清抗Dsg3抗体水平能反映寻常型天疱疮患者口腔损害的严重程度,可用于监控病情和指导治疗方案的调整,提高诊疗水平。

Abstract: Objective    To investigate serum level of anti-desmoglein 3 (Dsg 3) antibody and its influencing factors in pemphigus vulgaris(PV)patients. Methods    Totally 30 patients with pemphigus vulgaris diagnosed in Department of Oral Membrane Disease of Nanjing Stomatological Hospital from June 2017 to December 2020 were collected. The distribution of oral lesions was examined,the severity of oral lesions was evaluated,and serum samples were collected. ELISA was used to detect the level of anti-Dsg3 antibody related to pathogenicity in serum samples,and influencing factors of serum anti-Dsg3 antibody level were analyzed. Results    The level of anti-Dsg3 antibody was(116.52 ± 7.35)U/mL in patients with pemphigus vulgaris. Anti-Dsg3 antibody levels were not associated with age or gender in pemphigus vulgaris patients,nor with sites of oral lesions(P > 0.05),but were associated with the severity of oral lesions(Z = -3.070,P = 0.002). Conclusion    The serum level of anti-Dsg3 antibody can reflect the severity of oral lesions in patients with pemphigus vulgaris. It can be used to monitor disease development and guide the adjustment of treatment regimens,so as to improve the level of diagnosis and treatment.
