不同矢状骨面型下颌联合形态与下切牙角度和位置的特点及相关性研究#br#

摘要: 目的    分析不同矢状骨面型下颌联合形态与下切牙角度和位置的特点及其相关性。方法    选取2018—2020年于山西医科大学附属第一医院口腔正畸科就诊的错牙合畸形患者60例,根据ANB角(上牙槽座点、鼻根点与下牙槽座点构成的角)分为骨性Ⅰ类、Ⅱ类和Ⅲ类错牙合畸形患者各20例。通过锥形束CT(CBCT)分析不同矢状骨面型患者的下颌联合形态[包括下颌联合高度(Id-Me)、下颌联合凹陷度(Id-B-Pog)和下颌联合突度(B-Pog-Me)]、下切牙角度(IMPA)及下切牙根尖前后骨量(LA和LP)的差异。结果    不同矢状骨面型患者Id-Me、Id-B-Pog、IMPA、LP比较,差异均有统计学意义(均P < 0.05)。与骨性Ⅰ类和Ⅱ类错牙合畸形患者相比,骨性Ⅲ类错牙合畸形患者的Id-Me、Id-B-Pog较大,IMPA和LP较小;骨性Ⅱ类错牙合畸形患者的LP大于骨性Ⅰ类错牙合畸形患者;以上差异均具有统计学意义(均P < 0.05)。其他测量指标比较,差异均无统计学意义(均P > 0.05)。此外,IMPA与Id-Me、Id-B-Pog呈弱负相关性(r值分别为-0.384、-0.262,均P < 0.05)。结论    不同矢状骨面型下颌联合形态与下切牙角度和位置各异,下颌联合形态与下切牙角度之间存在相关性。

Abstract: Objective    To analyze the characteristics and relationship of mandibular symphsis and lower incisor angulation and position in different sagittal skeletal types. Methods    A total of sixty malocclusion patients treated in the Shanxi Medical University Stomatological Hospital from 2018 to 2020 were recruited. They were classified into Class Ⅰ,Class Ⅱ and Class Ⅲ skeletal malocclusion (n = 20)based on the ANB angle. The morphology of the mandibular symphysis [mandibular symphysis height(Id-Me),concavity of mandibular symphysis(Id-B-Pog),convexity of mandibular symphysis(B-Pog-Me)],the lower incisor angulation(IMPA),and the anterior and posterior bone mass of incisors(LA and LP)in patients with different sagittal skeletal types were analyzed by the cone beam computed tomography(CBCT). Results    There were significant differences in Id-Me,Id-B-Pog,IMPA and LP in malocclusion patients with different sagittal skeletal types(P < 0.05). Compared with those with Class Ⅰ and Class Ⅱ malocclusion,patients with Class Ⅲ malocclusion presented significantly larger Id-Me and Id-B-Pog,but smaller IMPA and LP(P < 0.05). LP in Class Ⅲ malocclusion patients was significantly larger than that in Class Ⅰ patients (P < 0.05). No significant differences were found in other indexes(P > 0.05). In addition,a weak negative correlation was identified between IMPA and Id-Me and Id-B-Pog(r = -0.384 and -0.262,respectively,both P < 0.05). Conclusion    The morphology of mandibular symphysis and the lower incisor angulation and position are different in different sagittal skeletal types,and there is a correlation between them.
