辅助根管冲洗器械对根管壁玷污层清理效率体外研究

摘要:

目的    研究4种根管冲洗系统对根管壁玷污层清理效率。方法    收集2016年1—4月于南京市口腔医院口腔颌面外科因正畸减数拔牙拔除的下颌前磨牙32颗,建立根管冲洗模型,随机分成4组,分别采用侧方开口针头注射器(A组)、Vibringe声波根管冲洗系统(B组)、RinsEndo根管冲洗系统(C组)和被动超声冲洗系统(D组)。预备并冲洗完成后,劈开根管,干燥喷金,扫描电子显微镜观察各区根管壁牙本质小管被玷污层覆盖情况。结果        玷污层评分情况:各组根管尖部均存在明显的玷污层覆盖牙本质小管,评分高于根管冠部和根管中部(P<0.05)。B、C、D组的根管中部、根管尖部评分低于A组(P<0.05)。4组在根管尖部根管内壁的玷污层评分差异无统计学意义(P>0.05)。结论    RinsEndo根管冲洗手机、Vibringe声波冲洗系统和被动超声冲洗系统在根管中部和根管尖部的玷污层清理效果显著好于侧方开口针头注射器的冲洗效果。

Abstract:

Objective    To study the effect of different irrigation regimens and methods of activation on smear layer removal in root canals. Methods    Thirty-two mandibular premolar roots were divided into 4 groups,with 8 teeth in each . In group A,the teeth were instrumented and irrigated by lateral opening syringe. In group B,irrigation was done using passive ultrasonic irrigation(PUI) system. In group C RinsEndo root canal irrigation system was used and in group D,Vibringe sonic irrigation system. Scanning electron microscope evaluation was done for assessment of smear layer removal in the coronal,middle,and apical thirds. Results    In each group,there was a smear layer covering the dentinal tubule in apical thirds,and the score was higher than that in the coronal and middle (P<0.05). The scores of B,C and D groups were lower than those in group A(P<0.05). Among A,B,C,D groups in the area of the root canal wall,difference of smear layer score was not statistically significant. Conclusion    Better removal of smear layer of the middle and apical thirds of root canal can be achieved by using RinsEndo,Vibringe and PUI system than by lateral opening syringe.
