胆囊癌新辅助治疗的难点与对策

摘要： 胆囊癌预后不佳，外科手术对于进展期胆囊癌治疗效果不尽理想，新辅助治疗模式能否改善进展期胆囊癌治疗效果成为关注热点。目前，胆囊癌新辅助治疗相关研究存在循证医学证据质量等级不高、概念混淆不清等难点。此外，开展胆囊癌新辅助治疗还应解决3个核心问题：（1）哪些病人需要接受新辅助治疗。（2）新辅助治疗采用何种方式。（3）新辅助治疗的疗效应如何评估。人工智能、类器官、液体活检等新技术的应用有助于为胆囊癌新辅助治疗带来新希望。

Abstract    The prognosis of gallbladder cancer（GBC）is very dismal and surgical resection renders little help for those at advanced stages. Whether neoadjuvant therapy can improve the outcome of GBC has become a hot topic of concern. However, there are still concerns regarding how to achieve effective new adjuvant therapy because of a lack of high-quality evidence-based data and misapprehension in clinical settings. In addition, three core issues need to be clarified, including the criteria of indication of applicable patientss, the specific treatment regimens, and the evaluation of therapeutic effect. The application of novel technological breakthroughs such as artificial intelligence,  an organoid and liquid biopsy may shed light on neoadjuvant therapy for gallbladder cancer.
