胆石症基础研究现状与展望

摘要: 胆石症系胆道系统结石形成及其所引发的感染、狭窄、梗阻等疾病,可引发腹痛、发热、黄疸等临床症状。近年随着我国代谢综合征病人和肥胖人群不断增多,胆石症在中老年人群中的发病率逐年升高。胆道微生物群的研究结果为胆石症防治提供了调整菌群的新思路。雌激素可促进内源性胆固醇合成。代谢异常如胰岛素抵抗可导致胆石症的发生。分子检测、细胞病理检测、影像学手段对于胆石症的检测有积极作用,应结合临床需求选择适合的检测方法。糖尿病、脂肪肝等是胆石症的危险因素,血清高水平HDL-C是新发胆石症的独立保护因素。合理控制饮食、增加食用水果和蔬菜可改变或降低胆石症发生的风险。同时,加强病人的饮食护理干预措施可有效预防和控制胆石症。

Abstract    Cholelithiasis is the common and frequent disease of gallstone formation and related complications including infection,stenosis and obstruction.It can cause abdominal pain,fever,jaundice and other clinical symptoms.In recent years,with the improvement of the living standard of Chinese residents,the number of patients with metabolic syndrome and obese people has been increasing,leading to the increasing incidence of gallstones in middle-aged and elderly Chinese.The study of biliary microbiota provides a new way to adjust the microbiota for the prevention and treatment of gallstone disease.Estrogen can promote the synthesis of endogenous cholesterol.Metabolic abnormalities such as insulin resistance can lead to gallstone disease.Molecular detection,cytopathological detection,and imaging methods have a positive effect on the detection of gallstone disease,and appropriate detection methods should be selected based on clinical needs.Diabetes,fatty liver,etc.are risk factors for cholelithiasis,and high serum HDL-C is an independent protective factor for new-onset cholelithiasis.Reasonable diet control and increased consumption of fruits and vegetables can change or reduce the risk of gallstone disease.At the same time,strengthening patients' dietary care interventions can effectively prevent and control gallstone disease.
