颈淋巴结清扫术易遗漏转移淋巴结部位及其对策

摘要:

甲状腺乳头状癌颈淋巴结转移较常见,规范的颈淋巴结清扫术有利于降低局部复发率及再手术率并改善病人预后。然而由于颈部解剖结构复杂,如果术前评估不仔细,清扫范围不规范,手术操作不熟练,某些部位的转移淋巴结容易在手术中被遗漏而导致残留。其中,位置较低的中央区淋巴结、右侧喉返神经深面淋巴结、Ⅱb区淋巴结、位置深在的Ⅳ区淋巴结、Ⅴ区淋巴结、颈丛神经根间淋巴结、胸锁乳突肌-胸骨舌骨肌间淋巴结和咽旁淋巴结是行颈淋巴结清扫术时容易被遗漏的转移部位,而颈部增强CT扫描有利于术前转移淋巴结的评估。熟悉容易遗漏的转移淋巴结部位、准确的术前评估、规范的清扫范围和娴熟的手术技巧是减少颈淋巴结清扫术转移淋巴结残留的关键。

Abstract    Cervical lymph node metastasis is common in papillary thyroid carcinoma (PTC). Standard cervical lymph node dissection is helpful to reduce local recurrence rate, reoperation rate and improve the prognosis for PTC patients. However, the anatomical structure of the neck is complicated. Metastatic lymph node is easily missed during surgery  without careful preoperation evaluation, standard dissection extent and skilled surgery technique resulting in the lymph node residual. Lower central lymph node, lymph node posterior to right recurrent laryngeal nerve, level Ⅱb, deeper lymph node of level IV, level V, lymph node between nerve root intersection, lymph node between sternocleidomastoid and sternohyoid muscle and pharyngeal lymph node are the most common missed locations during cervical lymph node dissection. Preoperative enhanced CT is helpful for the diagnosis and localization of metastatic lymph node. Familiar with the location of missed metastatic lymph node, accurate preoperation evaluation, standard dissection extent and skilled surgery technique are the key points avoiding lymph node residual during cervical metastatic lymph node dissection.
