胰腺实性假乳头状瘤CT、MRI征象与病理特点对比分析

摘要:

目的    探讨胰腺实性假乳头状瘤(SPT)CT、核磁共振成像(MRI)征象与病理特点的联系,以提高对该病的认识和术前诊断率。 方法    回顾性分析2004年4月至2011年4月广东省人民医院21例经手术和病理证实的SPT术前CT和MRI表现,并与病理结果进行对照分析。  结果    病灶位于胰头8例,胰体3例,胰尾10例。病灶最大直径3.5~18.0cm,平均6.3cm。9例以实性为主,8例为囊实性相间,4例以囊性为主;4例可见环形钙化,1例可见出血征象。2例恶性SPT中,1例出现肝转移,1例侵犯十二指肠壁。三期增强扫描所有病灶实性部分动脉期呈周边轻中度强化,门脉期和延迟期呈渐进性强化。 结论    SPT的CT、MRI表现能反映其病理特点,了解它们之间的关系有助于提高本病术前诊断率。

Abstract:

Abstract    Objective    To evaluate the CT and MRI findings of solid pseudopapillary tumor (SPT) of the pancreas with pathologic correlation and to improve the ability for the preoperative diagnosis of the disease. Methods    Preoperative CT or MRI images of 21 cases of histopathologically proven SPT in the pancreas admitted between April 2004 and April 2011 in Guangdong General Hospital were reviewed retrospectively and correlated to their pathologic features. Results Eight cases occurred in the pancreatic head,3 in the body and 10 in the tail. The size of SPT ranged from 3.5 to 18.0 cm (mean 6.3 cm). Tumors contained similar proportion of solid and cystic components in 8 cases, mostly solid components in 9 cases and mostly cystic components in 4 cases. Four lesions appeared calcification and one appeared hemorrhage. Of the two malignant SPT,one appeared hepatic metastases, and the other appeared invasion to duodenum. All of the lesions appeared mild or middle peripheral heterogeneous enhancement on arterial phase and progressive fill-in during the portal venous and equilibrium phases. Conclusion    CT and MRI images can provide the information about pathologic characteristics of pancreatic SPT. Diagnostic rate can be increased when knowing well with the relationship between them.
