Abstract    Endoscopic thyroid surgery refers to vidio-assisted or completely endoscopic thyroid surgery through a cervical or external approach. With the improvement of endoscopic technique, the accumulation of surgical experience and the development of instruments, the indications of endoscopic thyroid surgery have been extended to early papillary thyroid carcinoma.  Endoscopic thyroid surgery has the advantages of imaging magnification and postoperative beauty, but it also has some disadvantages. For example, when clean the lymph nodes in thyroid cancer endoscopic surgery , there may be blind areas in operation, leaving hidden dangers for tumor recurrence and metastasis. The main factors of thyroid cancer recurrence after endoscopic surgery are inadequacy of preoperative evaluation, non-standard operation and lack of endoscopic experience.  Tumor or lymph node “residue” is a common recurrence of thyroid cancer after endoscopic surgery.  The recurrence of thyroid cancer after endoscopic surgery should be carefully evaluated, and reoperation is often needed. Open surgery is advocated, and the reoperative principle is the same as the management of recurrence after open surgery.
