现代标准拔牙术新理念

  • 作者简介:胡开进，医学博士、教授、主任医师、硕士研究生导师。现任第四军医大学口腔外科主任。兼任中华口腔医学会牙槽外科学组组长，中华口腔医学会颞下颌关节病及牙合学专业委员会委员，中华口腔医学会颞下颌关节病学组委员，全军激光委员会委员，《中华实用医学杂志》常务编委，《牙体牙髓牙周病学杂志》、《实用口腔医学杂志》编委，《中华口腔医学杂志》审稿专家等职。2008年赴美国哈佛大学、南加州大学、加州大学洛杉矶分校等院校专门从事牙槽外科的参观和学习。主要从事颞下颌关节疾病及牙与牙槽外科疾患的临床、教学和科研工作。发表教学和科研论文100余篇（国外发表20余篇），出版专著20余部（主编及主译8部）。获得国家、军队、省部和院校级基金15项，获得国家、军队、省部级奖励13项。国家精品课程主讲教授、全军育才奖、第四军医大学十佳中青教师获得者。荣立三等功1次。

摘要：

提要：牙拔除术是口腔颌面外科领域最常见、最基本、应用最广泛的治疗性手术，也是口腔科医生必须掌握的一种基本治疗手段。传统的拔牙术，特别是不正规的操作或术前、术中及术后处理不当，易引起局部出血、感染、牙折、患牙被推挤移位，甚至颌骨骨折或颞下颌关节损伤等并发症。我们在学习国外高水平牙科医院的基础上，结合本科室的长期临床实践经验，总结出全新的牙拔除术“四化”理念——“标准化的器械”、“微创化的技术”、“规范化的操作”及“人性化的服务”。本文希望能为广大基层医院口腔外科医务工作者提供帮助，在巩固教科书讲授的基础理论知识基础上，对临床工作中的实际操作技术有进一步形象和准确的认识，能清楚地了解每项操作的潜在风险，更准确地了解自己的诊治能力和范围，提高诊治能力和操作信心。

Abstract:

 Summary：Tooth extraction is the most common treatment in the department of oral and maxillofacial surgery. It is also one of the most basic treatments which must be mastered by dentists. The traditional extraction technique，particularly inappropriate operation，could cause different complications，such as local hemorrhage，infection，tooth fracture，tooth displacement，and even jaw fracture or temporomandibular joint injury. Combining studies in dental hospital abroad with our long-term clinical experience，we summed up the new concept of modern standard tooth extraction，including standardized instruments，minimally invasive technology，normalized operation and personalized service. I hope this article could help oral surgeons consolidate the basic knowledge taught in textbooks and further understand the technology in clinical practice as well as the potential risk in each step. I also hope it could help oral surgeons improve their capabilities in diagnosis and treatment and enhance their confidence in operation.
