不同材料CAD/CAM髓超嵌体修复根管治疗后牙体缺损临床效果评价

摘要:

目的  评价计算机辅助设计与制造(computer aided design and computer aided manufacturing,CAD/CAM)新型纳米复合陶瓷Lava Ultimate髓超嵌体和长石质瓷Vita TriLuxe髓超嵌体修复根管治疗后牙体缺损的临床效果。方法 选取2017年1—12月于南京市口腔医院牙体牙髓病科就诊的58例患者根管治疗后缺损磨牙60颗,随机均分为两组,其中观察组30颗,采用Lava Ultimate髓超嵌体修复;对照组30颗,采用Vita TriLuxe髓超嵌体修复。根据改良美国公共卫生署(United States Public Health Service,USPHS)标准,比较两组临床修复效果。结果 60颗修复体均效果良好,无修复体脱落,仅在对照组发现1例修复体折裂情况;观察组在边缘密合性、完整性、继发龋和牙龈炎症方面与对照组比较,差异无统计学意义(P > 0.05);但观察组的对颌牙磨耗情况轻于对照组,差异有统计学意义(P < 0.05)。结论 Vita TriLuxe髓超嵌体和Lava Ultimate髓超嵌体皆可用于修复根管治疗后的牙体缺损,新型纳米复合陶瓷Lava Ultimate相较于长石质瓷Vita TriLuxe更能减少对颌牙的磨耗,值得临床推广应用。

Abstract:

Objective To evaluate the clinical effect of computer-aided design and computer-aided manufacturing(CAD/CAM) new nanocomposite ceramic material-Lava Ultimate endo-overlay and the feldspathic ceramic material-Vita TriLuxe endo-overlay in treatment of restoring tooth defects after root canal treatment. Methods Fifty-eight patients with 60 endodontically treated molars,who visited the Endodontic Department of Nanjing Stomatological Hospital from Jan. 2017 to Dec. 2017,were enrolled and divided into two groups. Lava Ultimate blocks were used to make the restorations in the 30 cases of observation group,while Vita TriLuxe blocks were used in the 30 cases of control group. The restorations of two groups were evaluated and compared according to modified United States Public Health Service(USPHS). Results All restorations displayed good results. None of the restorations fell off during the observation period except for one Vita TriLuxe endo-overlay in control group which fractured during follow-up. There were no statistically significant differences between observation group and control group in success rates of the marginal fitness,prosthesis integrity,secondary caries,or gingivitis(P > 0.05);however,the rate of paired jaw teeth wearing of observation group was lower than that of control group(P < 0.05). Conclusion Both Vita TriLuxe endo-overlays and Lava Ultimate endo-overlays can be used to restore endodontically treated tooth defects after root canal treatment,and new nanocomposite ceramics-Lava Ultimate can reduce wearing of paired jaw teeth compared to feldspathic ceramics-Vita TriLuxe,thus it is worth popularizing in clinical practice.
