  • 作者简介:唐晓琳，教授、主任医师、博士研究生导师。现任中国医科大学口腔医学院·附属口腔医院牙周病科副主任。兼任辽宁省口腔医学会牙周病学专业委员会副主任委员。目前主要从事牙周病学的临床、教学与科研工作。主要的临床科研方向为牙龈组织临床和生物学行为的研究。主持国家自然科学基金面上项目1项、其他国家级和省市级科研项目5项，参与完成国家、省、市及校级科研项目10余项。作为主要参与者获教育部科技进步奖二等奖1项、辽宁省医学科技奖一等奖1项、辽宁省科技进步三等奖1项。近年来发表学术论文40余篇，其中SCI收录10余篇。参编、参译著作2部。

摘要： Ⅳ期牙周炎以严重的牙周破坏和咬合紊乱为主要临床表现，并存在多种危险因素。通过病史回顾、临床检查、影像学检查即可鉴别Ⅲ期与Ⅳ期牙周炎。针对Ⅳ期牙周炎的临床特点，应强调多学科合作，并且在各个牙周治疗阶段注重咬合调整和咬合重建，以达到消除炎症、修复缺失牙、稳定咬合的治疗目标。文章围绕Ⅳ期牙周炎的临床特征、诊断与鉴别诊断和治疗计划加以概述。

Abstract: In the new classification of periodontitis，severe periodontal destruction and occlusal disorder are the main clinical manifestations of stage Ⅳ periodontitis. There are many risk factors for stage Ⅳ periodontitis. Stage Ⅲ and stage Ⅳ periodontitis could be distinguished by clinical examination，imaging examination and history review. According to the clinical characteristics of stage Ⅳ periodontitis，multidisciplinary cooperation should be emphasized in the treatment plan. And the occlusal adjustment and reconstruction should be emphasized in each phase of periodontal therapies. Only in this way can we achieve the goal of eliminating inflammation，repairing the missing teeth and stabilizing the bite. This paper reviews the clinical features，diagnosis，differential diagnosis and treatment plan of stage Ⅳ periodontitis.
