青蒿素衍生物蒿甲醚治疗口腔扁平苔藓40例疗效观察

摘要： 目的    探讨青蒿素衍生物蒿甲醚治疗口腔扁平苔藓（oral lichen planus，OLP）的疗效。方法    选择2017年12月至2018年6月在山东省妇幼保健院口腔医学中心就诊的OLP患者80例，随机分为试验组和对照组，每组40例。试验组患者口服蒿甲醚片（80 mg，1次/d）治疗，对照组患者口服硫酸羟氯喹片（200 mg，2次/d）治疗。比较两组的疗效、不良反应及复发情况。结果    治疗6周后，试验组总有效率92.3%，对照组总有效率95.0%，两组疗效的差异无统计学意义（χ2 = 1.439，P > 0.05）。治疗过程中试验组病例均未出现明显不良反应，而对照组病例有4例出现不同程度的不良反应。两组患者在治疗结束后随访6个月，试验组复发率（23.1%）低于对照组（33.3%），但组间差异无统计学意义（χ2 = 0.02，P > 0.05）。结论    采用蒿甲醚治疗OLP的疗效等同硫酸羟氯喹，不良反应少于硫酸羟氯喹，可作为硫酸羟氯喹治疗OLP的潜在替代药物。

Abstract: Objective    To explore the efficacy of artemisinin derivative artemether on oral lichen planus（OLP）. Methods    Totally 80 patients with OLP treated in Shandong Maternal and Child Health Hospital from December 2017 to June 2018 were included in this study. These patients were randomly divided into the experimental group and the control group，40 cases in each group. The experimental group was treated with oral artemether（80 mg，once daily），and the control group was treated with hydroxychloroquine（200 mg，twice daily）. The efficacy，adverse reaction and recurrence rate were compared between the two groups. Results    After a 6-week treatment，the total effective rate was 92.3% in the experimental group and 95.0% in the control group，and the difference in efficacy was not statistically significant（χ2 = 1.439，P > 0.05）. There was no case of significant adverse reaction in the experimental group，but there were 4 cases of different adverse reaction in the control group. The patients in the two groups were followed up for 6 months after the treatment. The recurrence rate of the experimental group（23.1%）was lower than that of the control group（33.3%），but the difference was not statistically significant（χ2 = 0.02，P > 0.05）. Conclusion    The therapeutic effect of artemether on OLP is equivalent to that of hydroxychloroquine，and the adverse reaction is better，which may be used as a potential alternative to hydroxychloroquine in the treatment of OLP in the future.
