沈阳市9 ~ 12岁学生第一恒磨牙窝沟封闭防龋效果评价及致龋因素分析

摘要:

目的    了解沈阳市9 ~ 12岁学生第一恒磨牙患龋状况,对其致龋因素进行分析并评价窝沟封闭项目2年后的防龋效果。 方法    2012年在沈阳市铁西区采用随机抽样方法选取3所小学共729名学生,将其中家长同意并签署窝沟封闭知情同意书的299名学生作为试验组进行第一恒磨牙窝沟封闭,其余430名作为对照组。封闭2年后复查两组学生第一恒磨牙患龋率,并结合问卷调查对致龋因素进行分析。结果    试验组窝沟封闭剂保留率为81.80%,患龋率为2.01%,明显低于对照组的16.51%,两组患龋率差异有统计学意义(P<0.05)。Logistic回归分析显示,是否进行窝沟封闭、刷牙频率和父母学历可影响学生患龋率。结论    窝沟封闭对第一恒磨牙防龋效果显著,应倡导尽早进行第一恒磨牙窝沟封闭,并加强对学生及其家长的口腔卫生保健宣传教育。

Abstract:

Objective    To analyze the risk factors and to evaluate the 2-year clinical anti-caries effect of pit and fissure sealant by investigating the prevalence of dental caries of the first permanent molar from 9-12 year-old students in Shenyang. Methods    Totally 729 students in Tiexi District were chosen from three primary schools,among whom 299 were selected as experimental group whose parents have signed the informed consent,while others were chosen as control group. Two years later,prevalence rate of dental caries in both groups were tested and risk factors were analyzed through combination with questionnaires. Results    The retention rate of sealant of the experimental group was 81.80%;the prevalence rate of caries was 2.01%,which was lower than that in control group(16.51%)(P<0.05),the difference being statistically significant. Logistic regression analysis showed that pit and fissure sealant(Y/N),the frequency of teeth brushing and parents’ education were positively correlated with the incidence of caries. Conclusion    Pit and fissure sealant is a protective method to reduce the incidence of dental caries. Both pit and fissure sealants and oral health education in early stage for children and parents should be advocated and strengthened to acquire a better oral hygiene.
