生长发育期唇腭裂患者的序列正畸治疗

  • 作者简介:范存晖，医学硕士、副主任医师、副教授、硕士研究生导师。1993年毕业于华西医科大学口腔医学院。现任职于青岛大学附属医院口腔正畸科。长期从事口腔正畸临床、科研及教学工作。擅长各类错牙合畸形的诊治，尤其是先天性唇腭裂的序列正畸治疗、口腔颜面畸形的术前及术后正畸治疗等。 2007年曾赴台湾长庚医院颅颜中心研修，学习唇腭裂序列治疗及颅颜畸形的正畸正颌联合治疗技术。学习归来后，开展了新生儿唇腭裂的鼻齿槽塑形术，临床诊治了大量的唇腭裂患者，取得了良好的疗效。致力于先天性唇腭裂的临床和科研工作，参与或主持多项科研课题的研究，发表论文30余篇，参编专著2部，获青岛市科技进步二等奖1项。

摘要：

唇腭裂是口腔颜面部最常见的先天性发育畸形，这类患者常伴随严重的错牙合畸形。正畸治疗是唇腭裂序列治疗的重要组成部分，贯穿于唇腭裂患者生长发育期的各个阶段。婴儿期鼻齿槽塑形术可重塑鼻、齿槽形态，为手术创造条件；替牙期是唇腭裂正畸治疗的关键时期，通过适时的植骨术前正畸治疗、扩弓和前方牵引等治疗，可以为尖牙的萌出创造条件，有效改善咬合及美观。

Abstract:

Cleft lip and palate is the most common craniofacial birth defect， which is often accompanied by serious malocclusion. As an important part of the treatment for cleft lip and palate， sequential orthodontic treatment is applied in all stages of growth and development. Presurgical nasoalveolar molding can improve the nasoalveolar profile of the cleft lip and palate in infancy to prepare for the future operation. Dentition is the critical orthodontic period to treat cleft lip and palate， which can facilitate the eruption of canines and effectively improve the occlusion and appearance through the orthodontic treatment before alveolus bone grafting， arch expansion and maxillary protraction in this period.
